Observation: Bar ice bin has a cold plate in bin. Ice must be separated from cold plate and tubing. Contact pop provider to provide a divider.

Observation: General overall cleaning is needed for establishment. Clean bar and kitchen and maintain at frequency to prevent future accumulation of debris.
- Using a handwashing sink- operation and maintenance
Observation: Employee rinsed bleander in bar hand sink. Hand sinks are to be for hand washing only. Owner states blender sink was leaking so employees were using hand sink until other sink could be repaired. Blender sink is now repaired. Educate employees on proper hand washing.
- Cleanability of floors, walls, and ceilings
Observation: Flooring in the walk-in freezer continues to deteriorate, the floor is buckling. Flooring must be smooth, nonabsorbent, and easily cleanable. Discussed with owner to grind the concrete down and proceed with plan of action once it is determined what is able to be done. --Observation: 1) Cove base is required at floor wall juncture to aid in cleaning. Install as needed. 2) Soiled ceiling tiles in employee RR. Replace as needed.
- Protecting food from environmental contamination
Observation: 1) Pop gun holder drain line was not attached and is mounted over ice bin. Gun could contaminate ice for drinks. Reattached. CORRECTED. 2) Ice bin at server station and bar are not covered. When not in use ice bins must be covered. Covers found and fixed. CORRECTED 3) Cups are being stored next to server hand sink. Food and food items must be protected from contamination. Move cups or install a splash guard.
- In-use utensils, between-use storage
Observation: 1) Using basket as a scoop for chips. 2) Handle for scoop in beans is in product. In use utensils must have a handle and be stored with the handle inverted. Replace basket with scoop and ensure handles are inverted.
- Manual and mechanical warewashing equipment, chemical sanitation - temp, pH, concentration, and hardness
Observation: Employee using only water on kitchen line for food contact surfaces. All food contact surfaces must be sanitized with an approved sanitizer. Bucket of QA sanitizer was made for kitchen line and employee educated on requirements.

Observation: Every hand sink to be used by employees must have a sign posted reminding them to wash their hands. Post signs.

Observation: 1) Visible mold growth on ice machine chute. Empty, wash, rinse, and sanitize and maintain cleaning at frequency to prevent future growth. 2) Pink slime in pop gun holder. Clean.

Observation: Wiping cloths laying on kitchen line. In between uses wiping cloths must be maintained in sanitizer. CORRECTED.
- Cooked PHF/TCS food cooled from 135°F to 70°F within 2 hours and from 135°F to 41°F or below in 6 hours
Observation: Refried beans in 6" deep pans located in the walk-in prepared on 3/31/15 (the day before) were temped at between 48-55 degrees. All food must be cooled from 135 to 70 degrees w/in 2 hrs and 41 degrees within additional 4 hrs. Beans were discarded. CORRECTED.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation: All cold holding equipment must have a thermometer available. Thermometer needed in salsa cooler.

Observation: No soap available at bar hand sink. Soap must be available at every hand sink. CORRECTED.
- Ice used as exterior coolant not used as a food
Observation: Can of pop was being stored in the ice machine ice bin. Ice that is used as a coolant is prohibited to be used as an ingredient. Can removed and ice machine to be emptied. CORRECTED.

Observation: Cooling refried beans in 6" deep pans. Cooling food should be done in shallow pans, use of ice bath, or purchase a blast chiller. Staff was educated.

Observation: Plumbing on dish area hand sink is leaking on to ground. All plumbing must be maintained in good repair. Repair as needed.

Observation: Condensate lines on walk-in cooler and freezer were stolen. Replace drain lines in accordance with manufacturer requirements. --Observation: Gasket on salsa cooler is detached and in poor repair. On speed rack in the walk-in cooler raw hamburger was being stored ove fresh cut veggies. All food must be stored in a manner that will prevent contamination. Product moved during inspection. CORRECTED.

Sate approved "No Smoking" sign must be posted at every entrance and exit to be used by the public. Given window clings during inspection. Post as needed.
- Roasts held at a temperature of 130°F or above
Currently hold salsa crafts at room temp due to no longer having a cooler by chip warmer. Salsa is a potentially hazardous food and must be maintained at 41 degrees or less. Employee to bring in a college dorm style frige until commerical cooler can be replaced.
- Miscellaneous sources of contamination
Containers on steam table and prep cooler were not covered. Containers in the walk-in are not all covered. When not in use all food must be protected from contamination. Lids were provided. Keep food covered when not in use.
- Cleanability of floors, walls, and ceilings
1) Some tiling missing in kitchen area. 2) Several large cracks in the walk-in freezer flooring. Flooring must be smooth and easily cleanable. Repair as needed. 3) Ceiling tiles in basement have water damage and must be replaced. 4) Wall in basement next to stairs is still open from water damage noted during routine inspection dated 4/30/2012. Repair wall.
- Hand drying provided

Bucket of sanitizer at the bar was in excess of 200 ppm. Chlorine sanitizer must be maintained between 50-100 ppm for proper sanitization. CORRECTED.
