Observation: All squeeze bottles of sauces, container of spices were not labeled with common name, need to label contents to prevent confusion. Must lable it. --Observed: 1) All food containers in the kitchen cooler and walk-in cooler were not labeled. All food taken from the original container and placed in another working container must be labeled with common name of the food contained. Manger agitated that this never been pointed, however it was on the report from 2013. This is REPEAT VIOLATION. Educated and verbalized understanding. Staff labeled CORRECTED on site during the inspection.
- Maintaining premises free of litter and unnecessary equipment
Observation: Lots of unused pieces of equipment, and panting material, are in the back storage area. The premises should be free of non-functioning equipment that is not in use.

Observation: One wet mop was laying against the wall on the floor. After use, mops shall be placed in a position that allows them to air-dry without soiling walls. Employee educated and verbalized. Corrected on site.

Observation: Quat test kit expired on 09/2014. Provide the proper test kit for testing the concentration of sanitizer solution.

Observation: Table in back area has foood residue. Msut clean top of prep daily to prevent build up and contamination.

Observation: One wet wiping cloth on 3 compartment sink and not held in sanitizer Wiping cloths used to clean food contact surfaces shall be maintained in sanitizer. Manager educated and verbalized understanding
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation: Date marking has improved and very good but one container with the cooked chicken in the cooler were not date marked Must date mark all TCS (time/temp control for safety) food with date opened/prepared. Corrected on site.
- Outer openings are protected
Observation: Gaps under or around exterior back-door-and has light showing through. Must seal gaps to prevent pests from entering. This is repeat violation. --Observation: The exit door does not shut all the way. Must have exit doors self closing and tight fitting to prevent entrance of pests and rodents. Repair as needed.

Has opened containers of pizza toppings meats without date marking, opened yesterday, must date mark and use within 7 days or discard remainder. Corrected on site.

1) Has ceiling tile missing in kitchen back area, reports has ceiling leak when rains, must get repaired. 2) Floor tile very cracked and several missing pieces of tile esp in area where gets water damage, replace to make floor smooth, cleanable surface.
