What is notice to employee as to change of relationship (Issued pursuant to provisions of Section 1089 of the California Unemployment Insurance Code)?
My company wants us to return this form to them along with severance package paperwork. I've never seen this form before. Is this a way to prevent unemployment benefits? Should I fill it out and send it back to them?
Its a standard form that puts in writing that your employment with the company has changed. In this case, it's because your employment is ending. It also notifies you that you are covered under California state's unemployment. Its standard (actually is technically required to be given to employees by employer when employment ends) but not often do you see companies abide by the regulations by providing one. Unless there's some weird non complaint language in the form, signing it is usually just an acknowledgement you're aware your employment is ending and you are covered by unemployment insurance.
