Three friends with cancer, one in the abdomen, one in the tonsil, one I think lymphoma, all were being referred to the mainland for care, two to MD Anderson, one to a practice in Seattle.

A one minute search online shows 44 oncologists in the state.

Of course Oahu has oncologists. But that is a broad stroke. I’m sure something like chemotherapy is broadly available. Surgical specialists seem challenging

This is no different than most any other place in the US, and is not all that unusual. Every population center has oncologists and cancer treatment centers. They treat the majority of cancer cases, because there are reasonably well established protocols for most cancers. The challenging cases, and people seeking clinical trials of other wise poor prognosis, get referred to major cancer centers around the country, and to places that are involved is specific clinical trials. MD Anderson is the top, or top 3 at least, cancer center in the US. Anyone needing that kind of expertise, or access to one the numerous clinical trials they are running will get referred there, regardless of where they live. That does not mean the area has "poor healthcare"
