I have a Samsung smart TV, almost three years old that does that occasionally. Always has. Depending on the app, sometimes just reversing a bit fixes it. Or fast forwarding. Sometimes I'll quit the app and start over. That almost always fixes it. Two ROKU TVs (cheap ones) do it much more frequently than the Samsung.
I've noticed recently that the Samsung is taking longer and longer to start up, and is noticeably slower responding to remote input... to the point of being aggravating.
Have you rebooted your TV lately? Hold the power button on the remote down until the TV turns off then starts back up.

Yep, I have a roku and audio seems to go out of sync fairly often when the commercials don't seem to load properly. I also keep the closed caption on because I am hard of hearing in one ear and lately have noticed a time or two that the closed captioning from whatever show I'm watching continues during the commercial. Talk about out of sync! Generally I just back out to the main screen, go back to my channel and "resume" the show and that usually syncs everything back up.
I just learned something, an hour ago, while talking with a Spectrum customer service rep. Seems I have wi-fi available, and didn't know it, as most of my TVs aren't compatible with it, so they wouldn't be able to link up with it, therefore I didn't explore that option. The rep suggested, just for grins, to swap my current cable box, since her data indicated that it was a little old, and if that didn't help, try logging that TV on to the wi-fi feed.
I have an appointment at the Spectrum store, in about an hour, to exchange the cable box, so that's what I'll start with.....
Update.....installed new cable box, but of course, the new layout of the various functions is TOTALLY different, and somewhat more complicated, so it will take getting used to.
The good news is that the "lip-sync" issue seems to be fixed, so far. However, I can't get the remote controller to program properly. It will change the channels, and scroll the functions, but it won't allow me to adjust the volume of the TV. The set-up sheets gives four different television ID codes, to try to use for the set-up. None of then allow me to do what's necessary.
What a fiasco this has turned into...................

What is the model of the new cable box? Also, does the TV appear to be reacting at all to what you have tried for codes?
Assuming not Xumo (streaming box), but if it were:
With a Xumo box, the connection method between the cable box and TV is via an HDMI cable plugged in between the two - generally referred to as CEC (Consumer Electronics Control).
If this function is turned off in the TV, the cable remote will not usually work with the TV.
