  • Basic - Case/container/bag of food stored on floor in walk-in cooler/freezer. Bag of potatoes and box of pre cooked sliced beef.
  • Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Multiple ice bucket 1 hanging from cord in front of ice machine the other on top of the soda syrup boxes at the soda syrup rack.
  • Basic - Walk-in cooler gaskets soiled with slimy/mold-like build-up. **Repeat Violation**
  • Basic - Wall soiled with accumulated food debris. Walk in cooler walk has red syrup running down wall behind storage rack.
  • High Priority - Employee failed to wash hands before putting on gloves to work with food. Employee at sandwich make table put on gloves the went to small white deep freezer got out frozen beef patties then put the patties on the griddle to cook. No hand washed.
  • High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Cream Cheese 60°F On shelf under the Good Creation Station. PIC stated the the cream cheese had only been on shelf for approximately 2 hrs. Corrective action PIC put cream cheese in walk in cooler.
  • High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature in chest freezer - all products not commercially packaged. Open bag of raw chicken directly on top of open box of beef patties in small white deep freezer next to Flat top griddle. Employee separated and stored items based upon minimum required cooking temperature. **Corrected On-Site**
  • High Priority - Toxic substance/chemical stored by or with food. Large white container of Super 4 Degreaser next to large container of fryer shortening on shelf under bun toaster at sandwich make line. PIC removed degreaser. **Corrected On-Site**
  • Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine. **Repeat Violation**
  • Intermediate - Accumulation of black/green mold-like substance inside the ice bin. Ice bin at drive through behind the Good Creations Station.
  • Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Sink at entrance to facility. Employee put paper towels at sink **Corrected On-Site**

  • Basic - Wiping cloth sanitizing solution stored on the floor. At drive thru.
  • Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine.
