The routine inspection report generated on 10/15/2014 has not been signed as there was no person-in-charge available at site at the end of inspection. The routine inspection was a joint inspection constituting inspectors from DCRA, Police and Pharmacy Control Division from DOH. The inspection resulted in a Summary Suspension of the license for violations of Title 25 of DCMR. Today's inspection is the continuation of the joint inspection. The following are requirements made to the operator in order for the license to be restored by DOH. - Come in person to the office to meet the program manager and area supervisory sanitarian. -Provide repair work invoice or report for the leaking condensate water line. - Produce a professional service report dated after the last DOH inspection has been conducted. - Pay the required permit restoration inspection fee to DOH - Continue cleaning the store - Provide a District of Columbia Certified Food Protection Manager I.D. card - Most Recent pest extermination service date: 09/27/2014 - No potentially hazardous foods (foods that need time and temperature) control observed during this inspection. - No expired food products observed at visit. - License not restored in this inspection. -If you have question or to schedule a meeting with Food Safety Program manager and supervisor, please, call the are supervisor Ms. Jacqueline Coleman at (202) 442 5928
