Correct response to questions There is no certified food protection manger on duty. Furthermore, there are concerns regarding the authenticity of the posted Food Safety Manager's certificate since it can not verified either by name of number in our system.
Management awareness; policy present The establishment did not provide proof of an employee health policy for the prevention of foodborne illness.
Proper use of restriction and exclusion The establishment did not provide an employee health policy regarding the prevention of foodborne illness.
Hands clean and properly washed An employee wearing a short sleeved t-shirt was observed washing her hands for only 7 seconds and did not wash above her wrists.
Food-contact surfaces: cleaned & sanitized Food residue is encrusted on many of the cooking pans in use.
Proper cooling methods used; adequate equipment for temperature control The ambient air temperature of the walk-in refrigerator was measured at 52 degrees Fahrenheit. The walk-in freezer is stacked to the ceiling and door with products and there is very little to no room for air circulation in the unit.
Thermometers provided & accurate There are no visible thermometers in the coffin style freezers in the basement.
Insects, rodents, & animals not present There are numerous missing ceiling tiles in the basement. There is gap in the ceiling tiles adjacent to the ventilation hood near the handwashing sink. There is a gap in the ceiling near the 1st floor front door due to a broken ceiling tile. An opening exists at the ceiling wall juncture above a storage shelf on the 2nd floor dry food storage room. There is an opening in the ceiling in the office due to a sagging ceiling tile.

Insects, rodents, & animals not present A large roach and a baby roach were observed on the wall behind a storage rack adjacent to the basement steps.
Contamination prevented during food preparation, storage, & display Cabbage is being cut on a cutting board placed on a drain board between the dishwash machine and garbage disposal unit. Whole heads of cabbage are touching the molded areas of the wall above the drain board.

Gloves used properly An employee was observed rinsing off a pair of disposable gloves and continuing to utilize the gloves as she handled the utensils.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, & used The shelves in the cabinet above the 3-compartment sink have an exposed nonabsorbent surface. The wooden shelves beneath the prep table are chipped and the paint beneath the wood is exposed.

There are food stains on the seals of one of the coffin style freezers. The rubber gasket on the basement walk-in refrigerator door and inner part of the door has mold.
Hot & cold water available; adequate pressure There is no hot water (at least 110 degrees Fahrenheit) at the men's toilet room handwashing sink.

Garbage & refuse properly disposed, facilities maintained The outdoor waste storage area is filthy and flies are buzzing around the dumpsters. A foul odor is emanating from the area and liquid spills are running down to the sidewalk.
Physical facilities: installed, maintained, & clean The 1st floor walk-in refrigerator floor is unclean and a sticky substance is on the floor while the basement walk-in refrigerator floor has liquid spills. The carpet on the 2nd floor dry food storage area is unclean and a sticky substance covers a large dark stained portion of the carpet. Mold is old the wall above the garbage disposal unit (food waste grinder) and adjacent drain board. A syrup-like stain is on one of the 2nd floor cabinet shelves.
Physical facilities: installed, maintained, & clean The dishwash machine is out of order and apparently will not be in use. The toilet room in the basement is not in use and a number of items there are not in use.
Physical facilities: installed, maintained, & clean There are water damaged and deteriorating ceiling tiles in the basement. Some of the beams in the ceiling have water damage and debris from wood and ceiling tiles is on the floor in the areas where there are damaged tiles and beams.

The establishment's certificate of occupancy is not posted in this location.
This establishment lacks active managerial control and the overall sanitation conditions in conjunction with the general physical condition of the establishment places its clients, especially those who enroll those considered ''highly susceptible,'' and public health at risk. Therefore we deem it necessary to suspend operations of this establishment until they are in full compliance with our regulations.
SUMMARY SUSPENSION (In order for the license to be restored, a reinspection fee of $100 for normal business hours or $400 for nonbusiness hours must be paid and all items must be abated and approved by the Department of Health.)
