COS-discarded the container and placed the cheese in a new clean container. KDA_4_60111A - --On the utensil rack there were 3 plastic food grade lids with stickers left on them and stored clean. There was also one white round plastic food grade container with food debris on the outside rim and stored clean. On the wooden shelf to the right of the 3 compartment sink there were 3 plastic food grade cambros with stickers left on them and stored clean. The large Hobart meat slicer had dried food debris in between the crevices of the machine and on the blade. It was used earlier today but was stored as clean during the inspection. 4-302.14 - --The 3 compartment sink located at the bar utilizes iodine and there were no test strips available at the time of the inspection. (They do have Quaternary Ammonia and chlorine test strips). KDA_3_10111 - --In the reach in cooler across from the grill there was a package of colby jack cheese with mold growing on the cheese. COS-discarded. At the bar there was a bottle of Bacardi Gold with 2 small flies in the product and one bottle of Dewar's Blended Scotch Whisky (White Label) with 2 small flies in the product. COS-discarded. KDA_3_30211A1b - --In the reach in cooler (across from the grill) there was a ziplock bag of a ready to eat half of a chicken and a ziplock bag of a ready to eat ribs being store next to a plastic containers of raw shrimp and raw pork chops. COS-rearranged items. KDA_3_30412B - --The metal ice scoop handle was in direct contact with the ice in the ice bin behind the bar. KDA_3_30511A1 - --In the dry food storage area there was one bag of golden brown sugar being stored directly on the floor. KDA_3_50118A1 - --In the Superior make table there was a plastic food grade container of hard boiled eggs (shells off) with a date of 11/23/15 (held past 7 days). COS-discarded. KDA_3_50118A2 - --In the RIC (across from the grill) there was a ziplock bag of ready to eat chicken, ziplock bag of ready to eat ribs, a ziplock bag of sliced ham, a metal food grade container of BBQ baked beans, a metal food grade container of green beans, an open bag of cut salad and a foam to-go container of prime rib, all items without dates. The ribs, chicken, BBQ baked beans, green beans and salad were made/opened on 11/28/15. The prime rib was made on 11/27/15 and the sliced ham was on 11/24/15. There was also a plastic food grade container of individually wrapped bags of brisket, a container of baked potato salad, a container of sliced red potato salad and a metal food grade container of loaded potato salad all without dates. The brisket was made on 11/28/15, the baked potato salad and loaded potato salad was opened on 11/28/15 and the sliced red potato salad was opened on 11/27/15. COS-all items in the unit were date marked. In the WIC there was a aluminum single use container of green beans without a date. The cook said it was made on 11/28/15. COS-item was date marked. In the Saturn RIC there were metal food grade containers of Canadian bacon, ground beef, ground sausage, pepperoni slices and a bag of sauerkraut all without dates. The cook said it was all placed in the unit on 11/27/15. In the RIC (below the Saturn MT) there was an open bag of ready to eat chicken and a plastic food grade container of pizza sauce without dates. The cook said the chicken was opened on 11/27/15 and the sauce was placed in the unit on 11/28/15. In the Maytag refrigerator there was a plastic food grade container of sliced tomatoes and a plastic food grade container of hard boiled eggs (shells off) without dates. The person in charge said the tomatoes were cut on 11/28/15 and the eggs were made on 11/27/15. KDA_4_10111A - --In the WIC there was a plastic Sterilite non-food grade container being used to store cut salad (in direct contact with the container). COS-placed the cut salad in a food grade container. KDA_4_20211A2 - --On the utensil rack there was one rubber spatula with a deep crack on the edge and food debris was on it and stored clean. COS-discarded. There was a plastic lid on one of the crock pots with a chunk of plastic missing, it was cracked and silver duct tape was being used to hold it together. KDA_4_204120 - --There is water pooling at the bottom of the walk in cooler (water is dripping from the ceiling). There is also water pooling at the bottom of the Superior make table/RIC. KDA_4_50111B - --The lid to the RIF with the Jack Daniels label is broken inside. KDA_4_50112 - --The white cutting board on the Superior make table has deep scratches and a blackish tent in between the crevices. KDA_4_60111A - --On the wooden shelf to the right of the 3 compartment sink there were 9 plastic food grade cambros with stickers left on the outside and stored clean (can hold bacteria). On the utensil rack there were 20 round plastic lids with stickers left on them and stored clean. There was a glass measuring cup with sticker residue left on it and stored clean. There was a rubber spatula that had dried food debris in the crevices (can hold bacteria)-discarded. COS-moved to the ware wash area. The large Hobart meat slicer has dried food debris in between the crevices of the machine and the blade and stored clean. KDA_4_60111C - --There is dust build up on the fan blowers inside of the walk in cooler. There is grease build up on the hood filters above the fryers. KDA_5_20515B - --The base of the left faucet at the 3 compartment sink leaks when water is supplied. The handsink is slow to drain. KDA_6_20211A - --The light in the WIC does not have a protective cover. The light fixture in the dry food storage area does not have a protective cover. The light fixtures above the RIF and RIC (food prep areas) do not have protective covers. KDA_6_50112A - --There is grease build up on the ceiling and on the wall behind the fryers. KDA_7_10211 - --On the wooden base below the ice machine there were 2 working spray bottles with clear liquid and not labeled. An employee said they were both sanitizer. COS-labeled both bottles. KDA_7_20111B - --There was a working pump bottle of lotion being stored directly on top of the ice machine. COS-removed the bottle of lotion.
