  • KDA_3_30212 - --Located in the RIC under the MT, there was an unlabeled container of yellow liquid which the PIC identified as crazy bread spread. COS- labeled. Located on a shelf under the prep counter, there was a large unlabeled tub of yellow powder which the PIC identified as cornmeal. COS- labeled. Located on a rack above the prep counter, there were two unlabeled shaker containers of yellow powder which the PIC identified as cornmeal. COS- labeled. Located at the front prep table, there were four unlabeled shakers of light yellow powder, two unlabeled containers of yellow liquid, and an unlabeled container of green powder which the PIC identified as parmesan cheese, butter spread, and veggie seasoning, respectively. COS- labeled.
  • KDA_3_30412A - --Located at the MT, there was a cup with no handle stored in contact with the pepperoni, a scoop with a handle in contact with sausage crumbles, and a scoop with a handle in contact with bacon pieces.
  • KDA_3_30512 - --There are bottles of water stored in the restroom.
  • KDA_3_50116A2 - --Located on the front counter next to the crazy bread warmer, there was a box with 48 individual cups of pizza sauce in it at 66F. PIC indicated that they had been sitting out since 11:15 this morning. COS- discarded. There are 20 pre-made pizzas at 75F stored on racks outside of the WIC. PIC indicated that they pre-make the pizza and then if it not cooked within two hours it is discarded. There was no documentation for using time as a public health control. COS- moved to WIC, provided education on using time as a public health control.

  • KDA_4_60111A - --Located on a rack above the prep counter, there were two cutting boards stored as clean that were soiled with an accumulation of dried food debris. COS- washed. Located on a rack above the three vat sink, there were two plastic food container lids stored as clean that were soiled with dried food debris. COS- washed. Located on the clean dish rack, there were three plastic food containers stored as clean that were soiled with dried food debris. COS- washed.
  • KDA_4_60111C - --The inside of the pizza dough roller had an accumulation of food debris. The fan guards of the WIC are soiled with an accumulation of dust. There is debris on the bottom shelf of the pizza warmers. There is grease on the front and sides of the pizza ovens. There is an accumulation of dried food on the food catcher of the pizza oven. The seals of the RIC under the MT are moldy. The seals to the WIC are moldy. The bottom shelf of the RIC has an accumulation of dried food debris in it. There is an accumulation of dried food debris on the premade pizza pizza racks.

  • KDA_4_20211A2 - --The perforation tool stored as clean above the three vat sink used for rolling on the pizza dough had pieces of plastic missing from the blade. The metal edging is bent on the pizza roller stored on the food prep table were the dough machine is located. COS Items were discarded.
  • KDA_4_60111A - --There was a commercial can opener mounted to a food prep table located in the back kitchen area that had dried food debris on the blade from a previous use. COS Item was cleaned.
  • KDA_4_60111C - --There is dried dust buildup on the fan covers located in the WIC. There is dried grease and dust buildup on the hood filters and hood cavity located above the pizza over. There is dried food debris buildup on the shelf's above the food prep table located in the back kitchen area. there is rust buildup on the shelf's in the lower portion of the MT.
  • KDA_7_10211 - --There was a working spray bottle with a blue liquid being stored under the three vat sink with no common name on the bottle. PIC said it was glass cleaner. COS Chemical was marked.
