  • KDA_3_30212 - --Under the make table on the west wall, were two containers with a yellow liquid in them with out a common name. Person in charge said they were vegetable oil and deep dish oil. On the prep table where the crazy bread topping is added, there were four shakers, three with white powder and one with a black/white powder and a container with yellow liquid in it with out a common name. Person in charge said they were: parmesan cheese, Italian dressing and buttery garlic. COS-containers were labeled.
  • KDA_3_50114B - --In the make table on the bottom right side, there were three containers of pizza sauce at 44.7F, person in charge said it was made Friday. Pizza sauce made yesterday was 47.2F and 46.8F. There was sauce in the walk in cooler that was 37.3F that was made yesterday. Educated on proper cooling. COS-sauce was discarded.
  • KDA_3_50116A2 - --In the make table, cheese sauce was 45.7. Cooked sausage was 54.7F. Person in charge said it had been in there for two days. COS-cheese was discarded. Ambient of the make table in the middle was 39.9F. Ambient of the make table where the pizza sauce was located 35.F.
  • KDA_4_20211A2 - --Above the three compartment sink, one black lid and one black spatula had breaks along the food contact surface area. On the green shelf a plastic container had a break. COS-items were discarded.
  • KDA_4_60111A - --The commercial can opener blade had dried on food debris. On a shelf above the three compartment sink, there were two black lids and two white lids, on the green shelf were seven plastic containers stored as clean that were greasy to the touch. Also stored there as clean were four lids that had food debris on them. Hanging on a rack above the three compartment sink was a small ladle and a plastic spatula, stored as clean that had grease on the food contact surface. On the make table bottom shelf where the commercial can opener is located, there were five baking sheets stored as clean and stacked with dried on food debris on the bottoms of them. Educated on proper cleaning. COS-all of these items were placed at three compartment sink.
  • KDA_4_60111C - --The commercial mixer has an accumulation of white powder and debris on the non food contact surface. The racks the pizzas are sitting on to rise, have an accumulation of grease and dirt. The blue racks the clean dishes are stored on to air dry have an accumulation of grease and dust. All four sides of the pizza oven have an accumulation of grease and food debris. The racks in the wall in cooler, have an accumulation of grease and food. The water cooler, has accumulation of dirt on the buttons used to dispense the water. The racks on the North and the west wall above the prep table have an accumulation of food and grease. The pizza warmer on the left has an accumulation of food debris. The rack with the crazy bread sauce has an accumulation of food debris on the bottom wheel. The handles on the following units have an accumulation of grease and dirt: walk in cooler, make table, pizza warmers, the crazy bread warmer. The glass on the crazy bread warmer also has accumulation of grease and dirt. The rack at the end of the make table has an accumulation of food and grease.

The following areas have vents with an accumulation of dust: pizza cutting area, customer area, ware washing area. Under the green rack in the ware washing area there is Styrofoam cups, spray nozzles to plastic bottles and an accumulation of dirt. The base boards through out have an accumulation of dust. The ledge around the compartment sink has black mold. The floor around the commercial mixer and the dough runner have an accumulation of grease and dirt. The wall and behind mixer has dried on food debris. Under the hand washing sink in the ware washing area, there is dirt and food debris, it's also located behind the three compartment sink.

  • KDA_Defacto - --200PPM Quat in the three compartment sink. --Frozen food is frozen. --In the left side of the make table cooked bacon crumbles were 42.1F. In the middle section of the make table, pizza sauce was 40.5F. Reach in make table, cooked sausage 41.3F. In the walk in cooler, cooked wings were 35.6F. --pizza in the left warmer 168.3F. Pizza in the right warmer 137.8F. Hot holding, wings 152.8F. --pizza out of oven 170.8F
