Observation: Walls under three compartment has been damaged, there are huge wholes that are not covered easily cleanable and a cove base is required at the floor/wall juncture and base cover needs repair.

Observation: Store has bodily fluid kit but not established procedures for responding to vomiting and diarrhea events. Provided the info to post in store and corrected on site.

Observation: A new employee was not offered to read and sign Food Employee Illnesses Reporting Agreement. Employee read and signed, corrected on site.

Observation: Ice-cream chest freezer has ice build up.

Observation: Vents above coffee and tea machine have dusty build up on the ceiling vents.

Observation: All walk in cooler door gaskets are torn except the far left three doors. Must replace gaskets to ensure adequate temp is maintained.

Observation: Pop nozzles on the fountain machine have become soiled and must be cleaned.

Observation: Mop was observed bunched up in the top of the mop bucket. Must allow mop to air dry when not in use

Observation: Lights are not all shielded in the following areas: Island display cooler, sandwich warmer, and back dry storage. Must shield all lights in food prep areas to prevent contamination.

Observation: Drawers under coffee/tea area and walk in fan covers are both soiled. Must keep clean to prevent buildup of bacteria.

Observation: Observed chemical bottles hung on wire rack used for clean dishes. Must separate chemicals from clean equipment, food, and food prep areas.
