Eating, drinking, or using tobacco Observation:Observed open beverage in dish room and in waitress area. Observed cigarettes on a ledge above the line cooler.
Warewashing equipment, determining chemical sanitizer concentration Observation:Observed dish machine not properly sanitizing food contact surfaces. Instructed that all dishes need to be washed in 3 compartment sink. Manager called repairman for service.
Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours Observation:Observed little date marking on prep lines and cooler. This is a repeat violation.
Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods Observation:Observed raw eggs over the top of cooked carrots and potatoes in walk in cooler. Observed raw beef over the top of cooked noodles.

Cooked PHF/TCS food cooled from 135°F to 70°F within 2 hours and from 135°F to 41°F or below in 6 hours Observation:Observed chili that was prepared and started cooling at 10:00 am. At 11:00 am the temp was 53.4* and at 12:00 pm the temp was 51.3*.
