The man really needs a higher level of care but he is so cantankerous and refuses any kind of help. Currently home care aides are doing stuff for him but they don't seem to report that he needs to be moved. Either that or management is again not responding appropriately.

I can totally believe that. From what I've seen it's the really cantankerous and sometimes violent ones who refuse all help are usually the ones that have the worst health and hygiene problems that they do not want to have to deal with so they just let it slide and get worse.
