    Observation: Foods in the top of the two door prep ccooler by the front counter are being held at 45-60F. Records were available to show the cooler was serviced on 3/17/15, however it is not holding proper temperatures today. Sour cream on the top of the one door prep cooler is at 46F. It was discussed that it will be stored in the bottom of the cooler to insure proper cold holding.
    Correction: Maintain cold potentially hazardous foods at or below 41°F

    Observation: The grout between the floor tiles in near the walk-in freezer the kitchen is deteriorating and providing a gap for debris to accumulate.
    Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.

    Observation: The coving tiles by the walk-in cooler, the frosty machine by the drive-thru window, under the 4 compartment sink, under the food prep sink, and by the sink in the women's restroom are pulling away from the wall or are broken and missing.
    Correction: Repair/replace the base coving.

    Observation: The women's restroom door does not close tightly. The floor around the toilets in the women's restroom is soiled.
    Correction: Maintain restrooms in a clean condition.

    Observation: Sliced tomatoes and mayo in drive-thru prep cooler is cold held at 61-68°F. Raw hamburgers in the hamburger prep cooler at cold held at 44-46*. Lettuce and sliced tomatoes in the walk-in cooler are cold held at 48*. Pickles in the prep cooler by the service counter are cold held at 64*. The single door prep cooler in the service area is cold holding at 51*.
    Correction: All food must be removed from these coolers until the cooler is able to maintain 41* or lower. A repair person is scheduled to visit the facility today. Maintain cold potentially hazardous foods at or below 41°F. I will return tomorrow to verify the coolers are either not being used or have been repaired. Correct By: 06-Aug-2014
