Observation: Food is obtained from an unapproved source. There is uninspected venison in one freezer, and in another freezer is pizza from Old World Pizza, 5660 Bishop Avenue, Inver Grove Heights, MN 55076, according to Wally and a pizza box cover that was shown. The pizza is wrapped in plastic wrap with no label other than such namers as : Bacon, Sausage, Cheeseburger. (Photos taken). There is no other information available to verify is comes from an appropriately licensed facility for the meat product to be made/distributed/ sold from Minnesota facility to Wisconsin restaurant. The operator was told the facility was applying for USDA approval but does not have any information onsite. Referral will be made to Minnesota and Wisconsin inspection agencies. Meat product requirements were previously reviewed with Julie Austin.
Correction: Discard foods from unapproved source. Product removed from premise at inspection.
- Critical: PACKAGED AND UNPACKAGED FOOD - RAW OVER RTE-FOODS (corrected on site)
Observation: Raw meat is stored over ready-to eat items in the Kenmore cooler and in the walkin cooler.
Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Raw meat was moved at inspection.
- PERSON IN CHARGE - DUTIES
Observation: Review Wisconsin Food Code requirements related to food source,storage and consumer advisory.
Correction: The person in charge shall review with employees:
- THERMOMETERS - FOOD - ACCURACY
Observation: The available food thermometer does not go below 50 F for food temperature measurement. Increments are in 5 F.
Correction: Replace thermometer with one that has at least 2 F increments and can measure cold hold and cooling to 41 F.

Observation: The biscuit gravy is not date-marked.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.
- FLOOR AND WALL JUNCTURES, COVED, AND ENCLOSED OR SEALED
Observation: Floor and wall juncture in the men's room is not coved to provide a seal near the urinal.
Per Ellie and Wally the toilet room is scheduled for floor/wall work this spring.
Correction: Provide base coving and nonabsorbent material.
