Correction: Inform food employees of the requirements to report signs, symptoms and diagnosis of foodborness illness to the person in charge.
Critical: PLUMBING - BACKFLOW PREVENTION DEVICE - WHEN REQUIRED Observation: Plumbing system not provided with an air gap or backflow prevention at basement faucet connected to green hose
Correction: Provide air gap or backflow prevention device. Plumbling systems shall be provided with backflow prevention at each point of use in the food establishment.
CONSUMER ADVISORY Observation: No consumer advisory provided or the consumer advisory on menu is missing.
Correction: Provide a consumer advisory for animal foods served in a raw or undercooked condition.
EQUIPMENT AND UTENSILS - FOOD CONTACT SURFACES, CLEANABILITY Observation: The far ice bin at the bar cannot be used for drink ice because it is not code compliant and does not have an air gap within 30 inches. This bin may be used to keep bottles of beer cold,,, but the ice cannot be used in drinks or for human consumption.
Correction: Provide food contact surfaces that are smooth, free of sharp edges and are free from breaks and cracks.
HAND DRYING PROVISION Observation: No single-use toweling or other hand drying device available at handwashing sink for hand drying at the bar.
Correction: Provide single-use toweling or other approved devices at handwashing sink to facilitate proper handwashing.
CERTIFIED FOOD MANAGER – REQUIREMENT Observation: There is no Wisconsin Certified Food Manager for this establishment.
Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 90 days of beginning operations.
