Critical: PHF/TCS, COLD HOLDING (corrected on site) Observation: Hot dogs and brats in the bottom portion of the prep cooler were cold held at 43-48°F. The containers of the hot dogs and brats were in direct contact with the bottom of the cooler. The ambient air temp of the cooler was observed at 38F.
Correction: Store the containers on another shelf in the cooler or place another shelf directly on teh bottom to allow for better air flow. Monitor food in the cooler when stored on the bottom shelf. Maintain cold potentially hazardous foods at or below 41°F. The containers were placed on another shelf during the inspection. Correct By: 29-Dec-2015
PREVENTING CONTAMINATION FROM HANDS - BARE HAND CONTACT PLAN Observation: The Bare Hand Contact Plan that is in place (and approved) states that the Person In Charge will keep a daily log verifying that food employee handwashing is occurring when required. The daily log is not being filled out. The Person In Charge stated that corrective actions are being taken and documented in employee files, but not documented on the daily log.
Correction: Two options for corrective action are as follows: 1.) Follow the existing Bare hand Contact Plan and resume documenting on the Daily Log or 2.) Revise the Bare Hand Contact Plan and include another method of documenting that handwashing is occurring. If Option 2 is followed and the Plan is revised, contact Outagamie County Public Health for review and approval. Correct By: 22-Jan-2016
EQUIPMENT AND UTENSILS - GOOD REPAIR (corrected on site) Observation: The chest freezer is damaged on one side with insulation showing. Frost is accumulated on this edge of the freezer.
Correction: The Person In Charge stated that they plan to replace the freezer within the next few months. Repair equipment to good condition or remove from premise. Correct By: 22-Mar-2016

Correction: Maintain cold potentially hazardous foods at or below 41°F. Monitor food temps in this cooler today
PREVENTING CONTAMINATION FROM HANDS - BARE HAND CONTACT PLAN Observation: The bare hand contact plan is not being followed. Food employees working on Assembly were observed washing hands, but not using the nail brush as it is indicated in the Plan.
Correction: Review the handwashing procedure that is included in the bare hand contact plan with all food employees. Ensure all food employees are following the procedure that includes double handwashing and using the nail brush every time hands are washed (while working in the identified positions).
