Observation: There are single service boxes and condiments stored under the handwashing sink. There are apples, cheese, wraps, and juice stored under a tray used in WIC for the leak.
Correction: Store food in areas not exposed to splash, dust, or other contamination.
- EQUIPMENT AND UTENSILS - GOOD REPAIR (repeated violation)
Observation: The 2 door freezer as rust and the coating is chipping. There is also duct tape being used to hold the seal. A prep table by the sink has duct tape and raw plywood holding it together.
Correction: Repair equipment to good condition or remove from premise.
- SINGLE-USE AND SINGLE SERVICE ARTICLES - PROHIBITIONS
Observation: There are single service bags, paper, etc stored on the floor in the dry storage area. There are straws stored in a box under the soda machine.
Correction: Store single-service or single-use articles, clean equipment and utensils and laundered linens in areas that are not subject to contamination.
- LIGHT BULBS - PROTECTIVE SHIELDING
Observation: The light above the chemical storage area where tea pitches are stored does not have a shield.
Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.
- CLEANING, FREQUENCY AND RESTRICTIONS (repeated violation)
Observation: The following are soiled and have debris accumulated:
-The cabinent under the soda machine
-The ice shoot on the soda machine
-The walls of the walk in cooler have mold build up
-The tray that is used in the WIC for a leak
Correction: Clean the physical facilities at a frequency necessary to keep them clean.

Observation: A box of coldcut combo meat in the walk-in cooler has a bucket of water sitting on top of it, catching dripping water from the condenser.
Correction: Store food in areas not exposed to splash, dust, or other contamination.
- EQUIPMENT AND UTENSILS - GOOD REPAIR
Observation: Upright one door freezer is cracked on the inside, exposing insulation. Duct tape is to repair the two door freezer. Raw wood and duct tape are used to repair the stainless steel table in the back storage room.
Correction: Repair equipment to good condition or remove from premise.
- FLOOR AND WALL JUNCTURES, COVED, AND ENCLOSED OR SEALED
Observation: Floor and wall juncture in the storage room, behind shelving is not coved. The wall paneling behind the soda syrups is peeling away. Ledge below safe is peeling away and taped.
Correction: Provide base coving and repair walls as necessary
- CLEANING, FREQUENCY AND RESTRICTIONS
Observation: The following areas are visibly sloiled and need to be cleaned
-Walls in the walk-in cooler
-Floor and wall under warewashing sink
-Handles of the freezers
-Cabinet under soada machine
-Tabletop under soda machine
Correction: Clean the physical facilities at a frequency necessary to keep them clean.

Observation: The floor of the walk-in cooler at the entrance is cracked.
Correction: Maintain the physical facilities so they are in good repair at all times. Repair or replace the walk-in cooler floor so it is smooth, non-absorbent and easily cleanable.
