Correction: Provide air gap or backflow prevention device. Plumbling systems shall be provided with backflow prevention at each point of use in the food establishment. Correct By: 01-Jan-2015

Observation: The bagged/bulk chicken strips in WIC were stored on the floor.
Correction: Store all food items 6 inches above the floor.
- UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION
Observation: Pizza slicing tool submersed in water at front line is improperly stored - directly adjacent to soap dispenser and hand wash sink - soap splashes into water where pizza slicer is stored when dispenser is used.
Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off the floor.
