Observation: Bartender observed not washing hands after going outside to smoke.
Correction: Before working with food, drinks, clean equipment, utensils, and single-use or single service articles employees shall properly wash their hands.
- CONTROLLING PESTS - TRAPPING DEVICES
Observation: Evidence of a rat in basement. Live trap and spring traps are set. I was told that they have been trying to eliminate the rat for the last 2 weeks. No professional pest control service here. The baement is very unorganized and cluttered with unnecessary items, providing harborage for rodents.
Correction: Eliminate rat(s) from establishment. It is hereby ordered that you work with a professional pest control company immediately. Provide a copy of the contract to the West Allis Health Department. The basement must be completely cleaned and organized. Remove all unused and/or broken items not related to the operation of the bar. Remove accumulated bagged aluminum cans. Correct By: 15-Apr-2016
- THERMOMETERS - FUNCTIONALITY - INTEGRAL OR PERMANENTLY AFFIXED MEASURING DEVICE
Observation: Thermometer missing in walk-in cooler.
Correction: Install an accurate thermometer in the walk-in cooler.
- MAINTAINING PREMISES, UNNECESSARY ITEMS AND LITTER (repeated violation)
Observation: See violation regarding rat/basement.
Correction: Items that are unnecessary to the operation of the food establishment shall be removed from the premise.
- PUBLIC RESTROOMS - GOOD REPAIR AND SUPPLIED WITH TOILET TISSUE
Observation: Men's room - floor at base of urinal is not clean.
Bare wood piece attached on wall behind urinal.
Correction: Clean and maintain toilet room in a clean condition.
Remove piece of bare wood and replace with a metal or fiberglass piece (like what is on walls).

Observation: Floor behind bar is in poor condition. Repair floor.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
- MAINTAINING PREMISES, UNNECESSARY ITEMS AND LITTER (repeated violation)
Observation: There are bags of cans, and other unused items in the basment. Get rid of unused items and do not store bags of cans and other recyclables downstairs. This is the second year in a row for this violation. Correct it.

Observation: Inside ice machine in need of cleaning. Store ice scoop in ice with handle extended or in a protected container.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.

- INSECTS AND RODENTS - OUTER OPENINGS - SOLID, SELF-CLOSING AND TIGHT FITTING DOORS
Observation: Screen door (back door) need s floor sweep/weather stripping to prevent rodents from entering.
Correction: Make the door self-closing and tight fitting to prevent the entrance of insects or rodents.
- MAINTAINING PREMISES, UNNECESSARY ITEMS AND LITTER
Observation: There is a significant amount of unused items in the basement. Including bags of soda and or beer cans. Remove all unused items and recylcables. Clean floor.
Correction: Items that are unnecessary to the operation of the food establishment shall be removed from the premise.
