    Observation: Employee noted working in food preparation/cooking area without a hair restraint.
    Correction: Provide employees working in food preparation and cooking areas with effective hair restraint (hat, hair net, beard restraint, etc.).
  • IN-USE UTENSILS, BETWEEN USE STORAGE
    Observation: The ice cream scoops were improperly stored. The water in the crock pot was 120 f. Alternatives were discussed.
    Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F.
  • EQUIPMENT AND UTENSILS - GOOD REPAIR
    Observation: The Traulsen reach in cooler gasket is not maintained in good repair.
    Correction: Repair equipment to good condition or remove from premise.
  • SINGLE-USE AND SINGLE SERVICE ARTICLES - ORIGINAL CONTAINER (corrected on site)
    Observation: The single service spoon in the ice cream area are not protected from contamination.
    Correction: Store single-use articles in their original container or use other effective means to prevent contamination such as container with the food contact surfaces in the container.

    Observation: The back area needs to be cleaned.
    Correction: Clean the physical facilities at a frequency necessary to keep them clean.

    Observation: There is no Wisconsin certified food manager for this establishment.
    Correction: Provide a Wisconsin certified food manager and post certification in the food establishment within 90 days. Employees are signed up for class on June 30th. In addition to the Serv Safe certificate, a State of Wisconsin food manager's certification is required.
  • HAIR RESTRAINTS
    Observation: Employee noted working in food preparation/cooking area without a hair restraint.
    Correction: Provide employees working in the food prep/cooking areas with an effective hair restraint (hat, hair net, beard restraint, etc.).
  • APPROVED – PLUMBING SYSTEM
    Observation: A metal cover is covering a hole that previously was a sink prior to the dish machine. It is not tight and water goes to a large pan that catches the water and is dumped when full.
    Correction: Plumbing is available that had been previously used for the sink.
  • FLOORS, WALLS, AND CEILINGS
    Observation: There are missing floor tiles at back door.
    Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable. .
