HANDWASHING SINKS, INSTALLATION - NON-HAND OPERATED Observation: Faucet valves on 2 of the hand sinks are worn requiring the handle to be turned too far in order for the water to turn off.
Correction: Repair or replace the faucet valves so the wrist blade handles can be used as intended and be turned off conveniently without having to grasp the handles.
FLOORS, WALLS AND CEILINGS - CLEANABILITY Observation: The grout lines in the floors in several areas are worn and low causing unnecessary standing water and accumulation of debris.
Correction: Re-grout the tiles in the needed areas to provide floors that are smooth and easily cleanable..

Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment. Notify me at the Health Dept when certification is received.
CLEANING, FREQUENCY AND RESTRICTIONS Observation: Ceiling vent grates and ceiling tiles in the warewashing area noted with dust accumulation.
Correction: Clean the physical facilities at a frequency necessary to keep them clean.

Critical: RTE/PHF-TCS PROPER DATE MARKING (repeated violation) Observation: Balsamic Vinaigrette dressing is still not properly date marked. This has now been noted as a violation on four consecutive routine inspections. This product is labeled "Keep Refrigerated" by the manufacturer, which indicates it is a TCS/potentially hazardous food and the Wisconsin Food Code datemarking rules shall apply.
Correction: All product from this bulk container was ordered is ordered to be discarded. Begin dating the Balsamic Vinaigrette container when opened and use or discard the product within 7 days. Further enforcement action may be taken due to the fact this has not been addressed over the last 3 years.
