Observation: (REPEAT) The chlorine is not being used according to EPA registered label use instructions and is at 0 PPM.
Correction: Employees must be trained on how to detect when sanitizer is not being delived via mechanical warewasher. Machine must be checked periodically to ensure sanitizer is being delivered at the proper concentration. Fix warewasher immediately and until this is accomplished, employees must wash dishes using manual sinks using sanitizer.
- TIME AS A PUBLIC HEALTH CONTROL - 4-HOURS - NOT MARKED
Observation: Lettuce, tomatoes, onions on prep line using time as a public health control for 4-hours is not properly marked or identified with a discard time.
Correction: Food items shall be marked or otherwise identfied with a time that is 4-hours past the point in time when the food was removed from temperature control. Discard food item.
- APPROVED PROCEDURES - VARIANCE REQUIREMENT
Observation: Variance Plan for using time as a public health control for potentially hazardous foods on prep line is not being followed.
Correction: Make correction to current procedure/practice to align with approved variance plan.
- USING A HANDWASHING SINK - ACCESSIBLE
Observation: (REPEAT) Handwashing sink observed to be a dumping station for dirty dishes and ice during inspection and is unavailable for proper handwashing.
Correction: Keep handwashing sink free of obstructions and in working condition at all times to allow for frequent handwashing.
- WIPING CLOTHS - USE LIMITATION - CLOTHS FOR WIPING COUNTERS STORED IN SANITIZER
Observation: (REPEAT) Many wiping cloth used for wiping counters stored on counter tops and food contact surfaces.
Correction: Cloths used for wiping counters shall be stored in a sanitizing solution.

Observation: Ice machine is broken.
Correction: Repair equipment to good condition or remove from premise.
- FOOD CONTACT SURFACES - NON TCS/PHF -IMPROPER CLEANING
Observation: Ice bin has visible debris build up on interior plastic lip which comes into contact with ice.
Correction: Clean equipment any time contamination occurs, at least every 24 hours, or at a frequency according to manufacturer specifications.
- NON-FOOD CONTACT SURFACES - CLEANING FREQUENCY
Observation: (REPEAT) The handsink is not cleaned on a frequent basis.
Correction: Clean handsink on a routine basis to prevent accumulation of soil residues.
- HANDWASHING SINKS, INSTALLATION - NON-HAND OPERATED
Observation: Handwashing sink faucet handles are not adjusted properly for it to be properly used without using hands.
Correction: Adjust faucet handles so handsink can be used properly.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY (repeated violation)
Observation: There are several cracked coving tiles and floor tiles.
Correction: Repair tiles so that are designed, constructed, and installed so they are smooth and easily cleanable.
- CLEANING, FREQUENCY AND RESTRICTIONS
Observation: Floor under and around broken ice machine has excessive build up and food debris.
Correction: Clean the physical facilities at a frequency necessary to keep them clean.
