Observation: No single-use toweling or other hand drying device available at hall bar sinks for hand drying.
Correction: Provide single-use toweling or other approved devices at the hall bar sinks to facilitate proper handwashing.

Observation: No employees had proper hair restarints.
Correction: Provide employees working in food preparation and cooking areas with effective hair restraint (hat, hair net, beard restraint, etc.).

Observation: The ice scoop at the ice machine was not protected from contamination between uses.
Correction: Store ice scoop in a clean plastic bag. Replace bag frequently.

Observation: The walk-in cooler and freezer both have food stored on the floor and are disorganized.
Correction: Reorganize both the walk-in cooler and freezer. Keep all foods off of the floor.

Observation: 1. The metal racks inside of the walk-in cooler are rusted. 2. Wooden cabinets in kitchen
Correction: 1. Replace/recoat or repaint thw metal racks used inside of the walk-in cooler. 2. As a long-term project, wooden cabinets in kitchen must be removed and replaced with approved cabinets.
- NON-FOOD CONTACT SURFACES - CLEAN
Observation: The following areas are dirty and must be cleaned: 1. The inside of the small cooler behind the hall bar. 2. The inside bottom of the bar coolers in the hall bar. 3. The ice bin behind bar in hall bar. 4. The soda gun cups (holders) behind hall bar. 5. Counter underneath glass storage at behind hall bar.
Correction: Thoroughly clean and sanitize all of these areas and maintain them in clean condition.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY
Observation: The floor of the walk-in cooler is rusted and worn.
Correction: Replace floor with an approved, cleanable material.
- CLEANING, FREQUENCY AND RESTRICTIONS
Observation: The floor of the walk-in freezer has food debris.
Correction: Clean the floor at a frequency necessary to keep them clean.

Observation: Two gallons of milk in the walk-in cooler dated November 27th.
Correction: Discard both gallons of milk.

Observation: The faucet at the dish sinks in the kitchen is not working properly.
Correction: Repair/replace faucet so it works properly.
- WAREWASHING - CHEMICAL SANITIZER CONCENTRATION
Observation: The Hobart dishmachine in the kitchen is not dispensing sanitizer. Also the machine ran through wash cycle but did not go through a final rinse cycle (during which sanitizer must be introduced to rinse water.)
Correction: Repair the automatic dishmachine so that it runs through the proper rinse cycle and a minimum of 50ppm chlorine sanitizer is provided during that rinse cycle.

Observation: Spray bottle of degreaser not labeled with contents.
Correction: Label all spray bottles with contents.

Observation: Barb Suter's food manager certificate expired on August 6, 2013. This establishment must have one emplyee with a Wisconsin food manager certification.
Correction: Provide one employee with training/class to meet requirement within 90 days.

Observation: The kitchen has wooden cabinets, shelves, etc.
Correction: .All wooden cabinets and shelving, etc. Must be replaced with approved, commercial cabinets and equipment.

Observation: The metal racks inside of the walk-in cooler are rusted. Also the floor of this cooler is badly rusted.
Correction: Replace or recoat the metal racks inside this cooler. Also replace or recover the floor inside this cooler.
- CLEANING FLOORS, DUSTLESS METHODS
Observation: The floors behind bar bars are dirty and need to be cleaned. (under the bar).
Correction: Clean the area underneath the bars thoroughly.
