Observation: Milk, tomatoes, adn cut leafy greens were not date marked. These were all added to be considered potentially hazardous foods since last inspection. Owner was able to correct.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.

Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment.
Correction: Provide & post an original State of Wisconsin food manager's certificate. Application will be e-mailed to operator.

Observation: There is no Wisconsin Certified Food Manager for this establishment.
Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 3 months. Test is to be taken next week Monday.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY
Observation: The Wall and Ceiling in the baesment is not to code.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
