    Observation: No air gap provided on the food prep sink in the upstairs kitchen.
    Correction: Provide an air gap on water supply side to protect water supply.

    Observation: The shield on the interior of the ice machine in the basement storage room is visibly soiled.
    Correction: Maintain food contact surfaces in a clean condition.
  • CERTIFIED FOOD MANAGER – REQUIREMENT
    Observation: There is no Wisconsin Certified Food Manager for this establishment.
    Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 90 days or a fee of $150 may be charged. The class is scheduled for 6/2/15.
  • EQUIPMENT AND UTENSILS - NONFOOD CONTACT SURFACES - CONSTRUCTED
    Observation: The rack over the prewash sink is deteriorated with raw wood exposed.
    Correction: Provide non-food contact surfaces that are constructed of a corrosion resistant, nonabsorbent, and smooth materials.
  • EQUIPMENT AND UTENSILS - PROPER ADJUSTMENT
    Observation: The seal on the left door of the True 2 door freezer is torn.
    Correction: Equipment components shall be kept intact, tight, and properly adjusted.

    Observation: There is a box of napkins, a box of bowls and a box of clam shells stored on the floor in the basement storage.
    Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off the floor.
  • FLOORS, WALLS AND CEILINGS - CLEANABILITY
    Observation: The floor at the entrance to the lower bar has gaps around a replacement piece of flooring and there is a crack at the entrance to the kitchen.
    Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
  • FLOOR AND WALL JUNCTURES, COVED, AND ENCLOSED OR SEALED
    Observation: Floor and wall juncture behind the fryer and grill in the lower bar is not coved.
    Correction: Provide base coving.

    Observation: The sliced tomatoes and deli meats in the downstairs kitchen are not date marked. Cooked vegetables, meat roll, sauces in the upstairs kitchen WIC are not date marked.
    Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.

    Observation: A chlorine test kit is not available for checking sanitizer concentrations at the lower bar.
    Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.
  • FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR
    Observation: Two boxes of bags of chips are stored on the floor in the basement storage area.
    Correction: Store all food items 6 inches above the floor.
  • NON-FOOD CONTACT SURFACES - CLEAN
    Observation: The shelving unit across from the 3 compartment sink in the upstairs kitchen is soiled with food debris and dust.
    Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
  • UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - SELF DRAINING POSITION
    Observation: Metal pans in the upstairs kitchen on the rack across from the 3 compartment sink are stored with the food contact surface up and a small amount of food debris is noted in a few of them.
    Correction: Clean equipment and utensils shall be stored in a self-draining position to allow for air drying and covered or inverted.
  • LIGHT BULBS - PROTECTIVE SHIELDING
    Observation: The ceiling lights located in the downstairs kitchen are not shatter resistant.
    Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.
