Critical: RTE/PHF-TCS PROPER DATE MARKING (corrected on site) Observation: Soup and cole slaw are not date marked in the walkin cooler. The soup was in the cooler for 3 days and the slaw 1 day.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days. Foods were date marked immediately.
BUILDING CONSTRUCTION Observation: A cover was missing on a vapor resistant junction box in the walkin cooler.
Correction: Food establishments shall meet the wisconsin commercial building code. Replace the cover as needed.
POSTING OF PERMIT Observation: Current permit is not posted in establishment. The permit is posted in the office.

Critical: PHF/TCS, COLD HOLDING (corrected on site) Observation: Sliced tomatoes were found at room temperature on the counter in the cooks area.
Correction: Maintain cold potentially hazardous foods at or below 41°F. The list of items requiring refrigeration now includes cut vegetables.
Critical: PLUMBING - BACKFLOW PREVENTION DEVICE - WHEN REQUIRED Observation: There are no back flow preventers on the hose bibs by the pre-wash sink.
Correction: Provide air gap or backflow prevention device. Plumbling systems shall be provided with backflow prevention at each point of use in the food establishment. Barb reports that this was recently discussed and a plumber is due in soon to instal them.
POSTING OF CERTIFICATE Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment.
Correction: Provide & post an original State of Wisconsin food manager's certificate. Barb recently took the class and the application was sent to corperate to send to Madison yesturday.
