Correction: Change procedures and provide training to employees on proper use of toxic chemical following manufacturer's directions for use. Spray bottles were remixed to attain 200 ppm.

Observation: A Quat test kit is not available for checking sanitizer concentrations.
Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.

Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment.
Correction: Provide & post an original State of Wisconsin food manager's certificate. Sheila Scwartz and James Heslip attended the 9/18, 9/24-2013 course at NTC. Fill out the provided application and send to Madison to obtain the Wisconsin Certificates.

Observation: Homestyle dishwasher is not ANSI certified or approved by the department.
Correction: Unapproved equipment shall be removed from food service. homestyle dishwasher does not reach 180 F for sanitizing dishes properly
- FLOOR AND WALL JUNCTURES, COVED, AND ENCLOSED OR SEALED
Observation: Floor and wall juncture in kitchen is not coved.
Correction: Provide base coving and clean kitchen floor.

Observation: Homestyle refrigerator is maintaining 41 F or less.
Correction: Once homestyle refrigerator no longer maintains 41 F or less, the unit must be replaced with a commercial unit. Currently the walkin cooler is used to cool food rapidaly.

- CONTROLLING PESTS - INSPECTING PREMISE
Observation: Evidence of rodents in the utility room, bottom kitchen cabinet and behind refrigerator and freezer.
Correction: Clean up old rodent droppings and rountinely inspect facilities for insects and rodents. Exterminate if rodents are still present.
