Observation: Wood's upright freezer has excessive frost build up.
Correction: Defrost freezer.

Observation: Low mein noodles cold held at 47 degrees F.
Correction: Maintain cold potentially hazardous foods at or below 41°F. Product was placed in cooler from WIC 1 hour ago. It and all other potentially hazardous foods have been placed int he WIC. Owner called repair man to fix cooler immediately.
- Critical: RTE/PHF-TCS PROPER DATE MARKING (corrected on site)
Observation: All food items in walk-in cooler not labelled but made day of. Once left overs are kept they are placed in cooks reach-in and to be dated. Not all were dated.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.

Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment.
Correction: Provide & post an original State of Wisconsin food manager's certificate. Class certificate was posted.
