Observation: The True 1 door prep cooler is not equipped with a thermometer.
Correction: Equipment used for hot and cold holding shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device.
- EQUIPMENT AND UTENSILS - PROPER ADJUSTMENT
Observation: Walk-in cooler door is not properly adjusted, intact or tight. The metal on the interior of the door is peeling away.
Correction: Equipment components shall be kept intact, tight, and properly adjusted.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY
Observation: Tiles on the walls throughout the facility, especially under the handwash sink, are broken and missing.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
- FLOOR AND WALL JUNCTURES, COVED, AND ENCLOSED OR SEALED
Observation: Coving tiles are missing, peeling away from the walls throughout the facility.
Correction: Provide base coving.
- CLEANING, FREQUENCY AND RESTRICTIONS
Observation: The wall below the three compartment sink and the floor below the fryers are soiled with food debris, grease and dirt.
Correction: Clean the physical facilities at a frequency necessary to keep them clean.

Observation: Pre-packaged cheese cups and sliced apples are being cold held at 47-48°F in the top portion of the one-door preperatory cooler.
Correction: Maintain cold potentially hazardous foods at or below 41°F

Observation: The plumbing system is not properly designed, constructed or installed. The indirect waste plumbing on various ice storage bins in the establishment does not terminate above waste plumbing with the required air-gap. In addition, the food preperatory sink indirect waste plumbing is installed via air-break and should be air-gap.
Correction: Design, construct, install, and maintain the plumbing system to conform to the State Uniform Plumbing Code SPS 382, 384. Correct By: 01-Jul-2014

Observation: There is no Wisconsin Certified Food Manager for this establishment although multiple employees have passed the "ServeSafe" food safety examination. Applications were provided
