CERTIFIED FOOD MANAGER – REQUIREMENT Observation: (REPEAT) There is no Wisconsin Certified Food Manager for this establishment. This is a 3 time repeat violation during the past 3 consecutive routine inspections. A $150.00 fee will be imposed the permit at this time.
Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 30 days. A chargeable re-inspection will occur in 30 days for this violation. This re-inspection will be charged at $578.00. If a Wisconsin Certified Food Manager Certification is not obtained by the re-inspection step enforcement will continue.
