Critical: FOOD CONTACT SURFACES - WAREWASHING CHEMICAL SANITIZATION EXPOSURE TIME Observation: Utensils in the mechanical warewashing operation are not exposed to the keystone sanitizer for the required amount of time. Test paper showed a 0ppm test result.
(CORRECTED DURING INSPECTION) Employees will use the 3-compartment sink to sanitize utensils after they are sent through the mechanical dishwashing operation with a 100ppm bleach solution. Owners contacted repair service immediately.
Correction: Adjust warewashing operation procedures or equipment to assure utensils are exposed to the chemical sanitizer solution for the required time period.
Critical: TOXIC SUBSTANCES - SANITIZER CRITERIA Observation: A bleach-dishsoap solution is being used in the food establishment to sanitize food contact surfaces and is not approved.
(CORRECTED DURING INSPECTION) Solution mixture of soap and bleach was discarded and a bleach-water solution of 100ppm was prepared.
Correction: Sanitizer shall meet the requirements under 40 CFR 180.940.
CONSUMER ADVISORY (repeated violation) Observation: The consumer advisory on menu is missing the disclosure statement reminding which particular items pertain to the reminder statement noted at the bottom of the menu.
Correction: Provide a consumer advisory for animal foods served in a raw or undercooked condition.
LIGHT BULBS - PROTECTIVE SHIELDING Observation: Lights located in the area before you go into the walk-in cooler and the single bulb above the salad prep area are not shatter resistant.
Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.

Critical: PACKAGED AND UNPACKAGED FOOD - NESTING OR RAW ANIMAL FOODS STORED TOGETHER (corrected on site) Observation: Raw beef and pork cuts are stored above fish (walleye, perch, etc.) in the walk-in cooler.
Correction: Prevent nesting of raw animal foods and store raw animal foods in order of descending hazard to prevent cross contamination. Separate raw animal food species from one another except when combined as ingredients.
CONSUMER ADVISORY Observation: The consumer advisory description is present at the bottom of the menus, but the reminder asterisking is not present on the specific items are not noted.
Correction: Provide a consumer advisory and asterisking for animal foods served in a raw or undercooked condition.
WAREWASHING MACHINES - AUTOMATIC DISPENSING OF DETERGENT AND SANITIZERS (corrected on site) Observation: The warewashing machine does not dispense sanitizer automatically. Management will use the approved 3-compartment sink for sanitizing their dishes until the sanitizer dispenser is repaired on their dishwasher.
Correction: Repair or replace the warewashing machine. Warewashing machines shall automatically dispense detergents and sanitizers. Dishes may be washed in the warewashing machine, but then a bleach-water dip at 100ppm shall be conducted and then drip dried.
