**CDC Risk Factor Violation 14 F**:
• Customer self-service soda station not clean to sight and touch.
4-601.11 Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils.
(A) EQUIPMENT FOOD-CONTACT SURFACES and UTENSILS shall be clean to sight and touch.
(B) The FOOD-CONTACT SURFACES of cooking EQUIPMENT and pans shall be kept free of
encrusted grease deposits and other soil accumulations.
(C) NonFOOD-CONTACT SURFACES of EQUIPMENT shall be kept free of an accumulation of
dust, dirt, FOOD residue, and other debris.
Violation 39 A
• There was no evidence of sanitizer in the damp wiping cloth storage container. One of the containers was very dirty. Violation was
corrected during the inspection.
3-304.14 Wiping Cloths and Working Containers, Use Limitation.
(A) Cloths that are in use for wiping FOOD spills shall be used for no other purpose.
(B) Cloths used for wiping FOOD spills shall be:
(1) Dry and used for wiping FOOD spills from TABLEWARE and carry-out containers; or
(2) Wet and cleaned as specified under ¶ 4-802.11(D), stored in a chemical SANITIZER
at a concentration specified in § 4-501.114, and used for wiping spills from FOOD-CONTACT
and nonFOOD-CONTACT SURFACES of EQUIPMENT.
(C) Dry or wet cloths that are used with raw animal FOODS shall be kept separate from
cloths used for other purposes, and wet cloths used with raw animal FOODS shall be kept in
a separate SANITIZING solution.
(D) Wet wiping cloths used with a freshly made SANITIZING solution and dry wiping cloths
shall be free of FOOD debris and visible soil.
Violation 52 E:
• There are no covers on the outside garbage containers.
• Cardboard storage is overflowing.
5-501.15 Outside Receptacles.
(A) Receptacles and waste handling units for REFUSE, recyclables, and returnables used
with materials containing FOOD residue and used outside the FOOD ESTABLISHMENT shall be
designed and constructed to have tight-fitting lids, doors, or covers.
(B) Receptacles and waste handling units for REFUSE and recyclables such as an on-site
compactor shall be installed so that accumulation of debris and insect and rodent attraction
and harborage are minimized and effective cleaning is facilitated around and, if the unit is
not installed flush with the base pad, under the unit.

A customer called to state his daughter and himself ate a bean burrito and became ill. The manager stated that they have not received any
other calls concerning illness from eating at their restaurant. She stated that none of the employees have called in sick. There is a sick
policy, which is hanging by employee hand wash. They also review the sick policy with the employees yearly.

Three compartment sink, food prep sink with air gap, mop sink, and employee hand wash sink present. Kay sanitizer used. The sinks and wiping
cloth buckets are were at 100 ppm. Test strips present.
The following temperatures were taken:
Bevles warmer: both were 150 f refried beans: 159 f & 174 f taco meat: 153 f & 154 f chicken: 162 f & 166 f
rice: 156 f & 143 f walk in cooler: 37 f Traulsen reach in freezer: 2 f 2 door prep cooler: 39 f
