    Observation: Sausage gravy found at 54.8 F in walk in. Breakfast was done at 10:30 AM. THis exceeded a 4 hour cooling time frame.
    Correction: PIC discarded. Manager to establish a process that cools this product within 4 hours.
  • Critical: Time as a Public Health Control* (corrected on site) (repeated violation)
    Observation: White, shredded and orange cheese for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time.
    Correction: Discard food that is not cooked and served or served if ready-to-eat within 4 hours from the point in time when the food is removed from temperature control. Clearly label or otherwise mark the food with a 4-hour cook-serve or serve by time. PIC discarded this product. Manager to review the time control process set forth by your company with staff.
  • Temperature Measuring Devices (repeated violation)
    Observation: There was no temperature measuring device located in the walk in.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Grills leak grease onto floor.
    Correction: This has been an ongoing problem. Grease dripping causes dirty floors and a mess that constantly requires good clean up. The grills were not made to leak onto the floor. Have this corrected.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: Door track to milk reach in is dirty.
    Correction: Maintain nonfood-contact surfaces of equipment clean.

Water found in bottom of milk display unit by drive through. Sometimes this is an indication that ice has formed inside and has melted causing the water issue. Keep an eye on this unit. Possibly drain may be clogged, as well. Stress the importance of discarding the food placed on time control and changing the times when new foods are placed on line.

    Observation: White cheddar cheese for which time rather than temperature is being used as a control was not cooked, served or discarded by the expiration time as indicated on the package.

Gaskets are ordered. Workers have had discussion about using the handsink after using the restroom. Sign was placed near handsink. Permit issued. Post in public view.

10/31/2014Follow-up
Keep permit in public view (it had fallen down). Post "No smoking" sign in conspicuous location. Keep large trash can away from front of handsink. Individual gogurts found in a pan on ice. A better way to keep cold is to add ice to the perforated pan you have the gogurt in. This was corrected. Leftover breakfast burritos were found at 49 F. These were discarded because they were not to be held after breakfast. Ensure this item is properly cooled after prep. Cover cup dispensers by drive through. Walk in freezer is very organized and clean. Fryers were found in good condition. Cheese was being time tracked. Permit expires 10/31/14. The above to be corrected. Call for reinspection.

    Observation: Observed worker return to kitchen station from restroom. No hands were washed in kitchen. Observed another worker leave station and proceed to put on gloves without washing hands.
    Correction: Both workers were told to wash hands.
  • Food - Packaged and Unpackaged - Separation, Packaging, and Segregation (corrected on site)
    Observation: Tea bucket found uncovered in walk in.
    Correction: Protect.
  • Cloths - Wiping Cloths - Use Limitation
    Observation: Wiping cloths improperly stored between use.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.
  • Food Storage - Clean and Dry Location (corrected on site)
    Observation: Magnesol was stored on cleaner shelf. Magnesol is a food additive that must be treated and stored as a food.
    Correction: This was relocated.
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: 1. Milk unit for coffee found at 61 F. 2. Low boy in back found at 44.9 F. 3. Canadian bacon found at 53.7 F in walk in.
    Correction: 1. Milk was discarded. New milk was put into unit and placed on a 6 hour time limit until unit can be serviced. 2. Food was relocated and a sign placed on outside not to use. Have serviced. 3. Canadian bacon was discarded. Manager to find out why this food is being abused during breakfast and correct the problem.

    Observation: There was no temperature measuring device located in several units.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: Loose panel on bun steamer. Door to ice machine is in poor repair. Several gaskets on refrigerators found torn. It appears there may be a leak in the grease drip tray for the flat top. Water found inside low boy where temperature was elevated.
    Correction: Secure panel. Make ice machine door smooth and cleanable. Replace split gaskets. Find leak for drip tray and repair.
  • Single-Service and Single-Use Articles, Use Limitation
    Observation: Cardboard box was reused to store prepared burritos.
    Correction: Cardboard is not cleanable and not intended to be reused.
  • Plumbing System Maintained in Good Repair
    Observation: Handsink along cookline is not operational due to clogged line.
    Correction: Handsink is essential to your operation. This is to be repaired as soon as possible.
  • Toilet Room Receptacle Covered
    Observation: There is no covered refuse container for the disposal of feminine napkins in the ladies room stall.
    Correction: Install a covered refuse container for the disposal of feminine napkins in the ladies room stall.

    Observation: Holes in wall above 3-c sink. Ceiling panels missing in storage area.
    Correction: Fill in holes so wall is smooth and cleanable. Replace missing ceiling panels.

    Observation: Floor drain under 3-c sink and floor behind flat top found dirty.
    Correction: Clean.

No risk factor violations observed. Gloves worn. Discussed need for carbonator backflow preventer to be installed on carbonator pump. 3 compartment sink - Quat sanitizer - 200 ppm - test kit (ok). Manager to survey use of drink cups on line and use of wet towels - need to be in sanitizer buckets.
