- 5.4 Keep the dumpster lids closed an clean the garbage surrounding the receptacle.
- 6.4 Clean the ceiling vent above the Monster beverage unit, which is dusty.
- 4.0/4.5 Clean or repair the rusty shelves holding the Monster beverages in the storage area, and throughout the walk-in cooler, to provide a cleanable surface.
- 4.4 Repair the damaged door seal on the entrance door of the walk-in cooler, to provide a cleanable surface.
- 4.0 Remove the cardboard lining the shelves in the walk-in cooler. Provide a cleanable surface or shelf liner.
- 4.4 Repair the taped duct taped handle to provide a cleanable surface.
- Notes: This operation received a huge order of bottled beverages for the holiday weekend. The order was received on the evening of 5/22/12, relocate bottled beverages in order to access the hand sink.
- Coffee was not for sale, and was offered for donation.

- 6.2 Provide 10 foot-candles of light throughout the walk-in cooler. Currently, 3.5-4.1 in the front aisle.
- NOTE: A beer cave was installed at this operation.
- NOTE: Observed the following foods for sale beyond the expiration date:
- -Sunflower seeds 11/14/10
- -BBQ Beef Jerky 8/6/10
- Check and rotate stock regularly.
- -Coolers are holding properly below 41°F.
