Observation:There was no detectable level of sanitizer in glass washing machine - bottle of sanitizer was replaced with fresh chemical, lines were primed and machine run through several cycles, corrected on site. Discussed ways to keep air from entering chemical container.

Observation:Observed boxes of fountain pop mix stored on basement floor.

Observation:Observed employee chop celery for tuna salad and handle the celery with bare hands, observed employee handle cooked fish and sandwich bun with bare hands - foods were discarded, corrected on site.

Observation:There was no detectable level of sanitizer in bar glass washing machine - there were large air bubbles in the line. Line was primed, corrected on site.

Observation:Observed openings along the bottom edges of both back entrances - daylight is visible.

Observation:There is no working mop hanger to hang the mop to dry.
- Indoor and outdoor surfaces
Observation:There are gaps visible along the wall panel seams in the walk-in freezer. Observed gaps around the water and waste water lines under the bar handsink and ice bin, at the floor.
- Equipment location, installation, repair, and adjustment
Observation:Observed openings around the condensation pipes inside the walk-in freezer - the insulation does not fully surround the pipes, causing ice buildup. Observed dust buildup on the back of the condensation unit in the walk-in cooler.
- Cleanability of floors, walls, and ceilings
Observation:Observed chipped and broken floor tiles around the mop sink and in front of the door to the walk-in freezer.
