Observation:Observed cases of food on walk-in freezer floor, and bag of onions and boxes of potato chips and soup mix on storeroom floor. Repeat violation.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation:There is no thermometer in kitchen refrigerator.
- Fish packaged using a reduced oxygen packaging method is frozen before, during, and after packaging
Observation:Observed thawed fish in vacuum packaging. Owner stated the fish is frozen and thawed for preparation, then thawed fish that is not used is vacuum-packaged on site and stored in the walk-in cooler. 5 packages of fish were voluntarily discarded, corrected on site.

Observation:Observed raw shell eggs and raw sausage stored with and above ready to eat foods in kitchen refrigerator and raw shell eggs stored with milk in walk-in cooler - corrected on site. Repeat violation.
