Forbes assembled a list of the 20 most profitable types of businesses, on a pretax basis. At No. 1: offices of Certified Public Accountants, with an average pretax margin of 16.5%. Offices of physicians (except mental health specialists), which clock an average 10.4% margin, brought up the rear.
From personal experience, these profit margins (for physicians' offices) seem low to me. In fact, surveys from Medical Economics show margins of 30 to 60% to be the norm (depending on specialty; low overhead specialties like radiology which don't require lots of staff have higher margins than primary care physicians).

Maybe chiropractors net profit is high as a percentage of the gross. Their gross revenue is very low. Also, a personal observation: most chiropractors I have dealt with were jerks or snobs.
Business consultants that operate out of their home have the best true profit I've seen. Attorneys have to impress their clients with a fancy car, office, etc. Most consultants that travel to meet clients don't have these expenses and most established ones have a great profit margin.
