Violation descriptions and comments

Peppers, oranges, and tomatoes in the back preparation area reach-in refrigerator were moldy.
Chicken in the front service area steam table was 114�°F.
No atmospheric vacuum breaker was available at the mop sink faucet.
An unapproved insecticide was being used in the kitchen area.
A chemical container was being stored on the food preparation sink.
Chemical spray bottles in the mop closet and ware washing areas were unlabeled.
Seasoning and other food containers in the kitchen area were unlabeled as to their contents.
Potentially hazardous foods were being stored in a non-commercial refrigerator in the back preparation area. Cutting boards in the preparation area were not smooth and easily cleanable. Foil was present on several utensils by the cooks' line grill.
The door gasket on the stand up reach-in refrigerator was torn. The food preparation sink was not completely sealed to the wall. The caulking around the hood was missing and/or in disrepair. Shelves in the walk-in refrigerator were rusty. The three compartment sink was not completely sealed to the wall. The shelves in the front service area were not smooth and easily cleanable. The storage shelf next to the cooks' line was lined with foil.
The interior of the back reach-in freezer was soiled. The interior of the microwave was soiled. Several cutting boards in the kitchen were stained.
The hood filters were soiled with grease. The shelves in the walk-in refrigerator were soiled. The top of the dish machine was soiled.
The handle of the scoop in the bulk food container by the kitchen entrance was being stored in contact with the food product.
Single use containers were being reused for food storage in the back preparation area.
The kitchen and ware washing hand sinks were not completely sealed to the wall. The drain pipe below the food preparation sink was leaking. The mop sink was not completely sealed to the wall.
Several areas of the kitchen ceiling were soiled and/or in disrepair. The concrete floor of the kitchen was not smooth and easily cleanable. Several areas of the floor in the dry storage, cooks' line and ware washing areas were soiled. The wall across from the oven was in disrepair. The wall behind the dish machine was soiled.
Light shields in the ware washing and preparation areas were missing.
