Shell fish tags were not retained for 90 days and were not in chronological order.
A bottle of bourbon was contaminated with a fruit fly.
A consumer advisory was not provided for foods served rare or undercooked.
Two employees were eating in the back kitchen area. Unapproved employee beverages were stored above the clean side of the dish washer, under the back food preparation table next to food service items, and above the beer coolers with food service items.
Bread pudding was 117�°F stored above the cook top.
Tomato pasta sauce on ice on the cook line was 47-55�°F. Cubed cheese on ice was 55�°F. Portioned cooked pasta with sauce and meatballs on the cook line was 46-48�°F, on top of the cook line oven was 89�°F, and in the walk-in cooler was 49�°F. Prosciutto in the specialty meat slicing area was 56�°F. On the pizza cold top ricotta was 49�°F, mozzarella was 44�°F, puree garlic in oil was 70�°F.
The bar hand wash sink lacked paper towels.
A spray bottle at the mop sink was not labeled.
A carton of tomato vegetable sauce was cooling tightly in a plastic container in the reach-in refrigerator by the cook line. Raw meat was thawing wrapped under running water in the food preparation sink. There were boxes of food on the floor in the walk-in freezer. The bar ice bin was not covered when not in use.
The interior of the ice machine was soiled with residue.
Pans were stacked wet in the dish area, not allowed to air dry properly.
Utensil on the cook line were stored in standing water at 87�°F and 123�°F.

Bread pudding was 70�°F in the reach in refrigerator next to the east cook's line hand sink. (Corrected on site)
