The purpose of today's visit was to conduct a routine inspection. Employee health policy in place. Sanitizer observed at acceptable level in the wiping cloth bucket, dishmachine, bar three vat sink, bar glass washer. Facility maintains temperature logs. Chefs observed using thermometers. Several employees observed washing hands during inspection using proper technique. The only outstanding violation is to obtain a letter of parasite destruction for the halibut, salmon, seabass and shrimp (in ceveche) which are served raw or undercooked and are noted as such on the menu. Within 10 days fax or email the letters of parasite destruction for each of the above fish. Thank you. If you have any questions, please feel free to call. Observation: The following fish offered for service or sale in the ready-to-eat form was not frozen to ensure parasite destruction: Salmon, halibut, seabass, shrimp.
Correction: Provide letters to the health department within 10 business days. Before service or sale in ready-to-eat form, raw or partially cooked fish shall be: 1) frozen and stored at a temperature of -4°F or below for a minimum of 7 days in a freezer, or 2) frozen at -31°F or below until solid and stored at -31°F or below for a minimum of 15 hours, or 3) frozen at -31°F or below until solid and stored at -4°F or below for a minimum of 24 hours.

The purpose of today's visit was to conduct a risk factor assessment inspection. Time reflects inspection time only.
- Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device:
1) Sliced turkey, diced tomatoes and whipped cream all at 45F in bottom portion of 2DR prep cooler,
2) Pre-cooked chicken wings at 50F in 3 drawers cooler.
Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. ITEMS UNDER #1 MOVED TO FREEZER TO RAPIDLY CHILL AND ITEM 2 DISCARDED.

Observation: Observed that inadequate lighting was provided in handwashing areas and toilet rooms.
