The purpose of htis visit was to conduct a risk factor assessment. Chlorine concentration wiping cloth bucket initial/final: 0/75 ppm. Chlorine concentration 3 vat sink: 100 ppm.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site)
    Observation: When tested, the chlorine-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the wiping cloth bucket with a concentration of 0 ppm total chlorine.
    Correction: A chlorine-based sanitizing solution shall have a minimum concentration of at least 50 to100 ppm total chlorine when tested using the appropriate test kit. NOTE: PIC adjusted chlorine conentration to 75 ppm.

The purpose of this visit was to conduct a routine inspection. Chlorine sanitizer 3 vat sink/wiping cloth bucket: 200/200 ppm. NOTE: PLEASE REMEMBER TO PROVIDE COPY OF CFM PHOTO ID'S AND COPY OF REVISED MENU TO HEALTH DEPARTMENT WITHIN 10 DAYS.
  • Critical: Molluscan Shellstock/Maintaining ID/Tags Attached until Container Emptied (corrected on site)
    Observation: Inadequate record keeping system for molluscan shellfish when removed from their tagged or labeled container.
    Correction: Shellstock tags shall remain attached to the container in which the shellstock are received until the container is empty. NOTE: Tags were located and placed with the mussels/clams in their individual containers. PIC will regularly check to verify tags are kept with shellstock until all are served or discarded, and then kept for minimum 90 days from havest date afterwards.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Sliced tomatoes/raw shell eggs:46/47 degrees F.
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. NOTE: Both foods were determined to be out of temperature <2 hours. Eggs were relocated to prep cooler bottom. Lid was closed on prep cooler top to facilitate cooling/ cold holding of sliced tomatoes.
  • Critical: Consumer Advisory, Disclosure Provided
    Observation: The following raw and/or undercooked foods are provided on the menu, menu board, table tent or brochure without proper disclosure: Carne asada, Tacos carne asada, and Salmon Parilla. All food items on menu were marked with an asterisk, so that specific reference to consumer adviosry warning is not provided. Also, no mention of foods being cooked to order or served raw or undercooked is provided.
    Correction: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived foods as raw/undercooked or identification of the animal-derived foods by asteriking (*) them to a footnote that states that the items are served raw or undercooked, or contain raw or undercooked ingredients. NOTE: PIC" advised new menus are currently being produce. Provide copy of new menu withy required consumer advisory within 10 days.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). NOTE: Provide copy of CFM photo ID to Health Department within 10 days.
  • Handwashing Cleanser, Available at Handwashing Facilities (corrected on site) (repeated violation)
    Observation: Observed that hand soap was not provided at each handwashing sink.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands. NOTE: PIC provided hand cleanser at hand sink.

    Observation: The tags for the molluscan shellfish are not available or are discarded immediately after the container is empty. TAGS FOR THE OYSTERS INT HE WALK-IN-COOLER WERE DISCARDED BY A NEW EMPLOYEE.
    Correction: Shellstock tags shall remain attached to the container in which the shellstock are received until the container is empty. WILL NOT SERVE THE OYSTERS.

    Observation: Ice used as a medium for cooling the exterior surfaces of food is also for beverage ice or used as food. OBSERVED TWO BOTTLES OF BEER IN THE ICE BIN AT THE BAR.
    Correction: After use as a medium for cooling the exterior surfaces of food, packaged foods, beverages, or cooling coils/tubes, the ice may not be used as food. REMOVED BEER BOTTLES.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked: ASCEND 2DR PREP COOLER (1) SHREDDED MOZZARELLA CHEESE (2) COLE SLAW (3) CEVICHE
    Correction: WALK-IN-COOLER (4) PICO DE GALLO (5) REFRIED BEANS
  • Critical: Consumer Advisory Required for Raw or Undercooked Animal Food
    Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food items that may be served raw and/or undercooked: (1) CARNE ASADA IN TACOS DE CARNE ASADA (2) EGGS IN HUEVOS RANCHEROS (3) CARNE ASADA IN ENCHILADAS (4) COSTILLA DE RES (5) SALMON A LA PARILLA
    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. INDICATE MENU ITEMS THAT ARE RAW OR UNDERCOOKED AND PROVIDE A DISCLOSURE AND REMINDER STATEMENT ON THE MENU. PLEASE SEND A COPY OF THE CORRECTED MENU TO THE HEALTH DEPARTMENT WITHIN 1O DAYS.

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. CFM ARRIVED DURING THE INSPECTION.

    Observation: Light bulbs in the ICE MACHINE AREA are not covered by a protective shielding.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles.

    Observation: The tags for the molluscan shellfish are not being retained for 90 days.
    Correction: The identity of the source of shellstock (oysters, clams, mussels) that are sold or served shall be maintained by retaining shellstock tags or labels for 90 calendar days from the date when the last shellstock from the container is sold or served using one of the following methods: 1) an approved record keeping system that keeps the tags or labels in chronological order correlated to the date when the last shellstock from a container are sold or served, 2) preserving source identification by using a record keeping system, and 3) ensuring that shellstock from one tagged or labeled container are not commingled with shellstock from another container with different certification numbers, different harvest dates, or different growing areas as identified on the tag or label before being ordered by the consumer. EHS went over the proper organization of keeping shellstock tags with CFM

    Correction: Food temperature measuring devices that are scaled only in Fahrenheit shall be accurate to +/- 2°F in the intended range of use.

    Correction: A chlorine-based sanitizing solution shall have a minimum concentration of at least 50 to100 ppm total chlorine when tested using the appropriate test kit. CFM reset solution to 50ppm

    Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.

    Observation: A food handler was observed drinking from an uncovered beverage container while in the kitchen.
    Correction: Food employees may drink only from a covered container using a straw. CFM DISPOSED OF BEVERAGE.

    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees.
