The purpose of this inspection was to conduct a routine inspection with focus on risk factors. Chlorine sanitizer in 3 vat sink/wiping cloth bucket: 75/200 ppm.
  • Critical: Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site)
    Observation: Open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils.
    Correction: Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items. NOTE: Discussed withPIC.
  • Critical: Time as a Public Health Control/ Up to 4 Hours for Cold & Hot PHF(TCS) / Requirements
    Observation: The following food item(s) for which time rather than temperature is used as a control is not marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control. Half and half creamer in carafe.
    Correction: If time only, rather than time in accordance with temperature control, up to a maximum of 4 hours, is used as the public health control the food shall: 1) have an initial temperature of 41°F or less if removed from cold holding temperature OR 135°F or above if removed from hot holding temperature control, 2) be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control, 3) be cooked and served, served if ready-to-eat, or discarded within 4 hours from the point in time when the food is removed from temperature control, and 4) be discarded if unmarked or marked to exceed a 4 hour limit. NOTE: Discussed with PIC.

    Observation: The door gaskets of the following unit(s) are (missing, damaged): 3 dr prep cooler left at cookline.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.

The purpose of this visit was to conduct a routine inspection. Chlorine sanitizer 3 vat sink/wiping cloth bucket #1/#2: 100/100/200 ppm. Discussed public health significance of proper date marking with PIC and provided date marking fact sheet.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: gallon milk/ half and half in 2 dr UC cooler: 47/48 degrees F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. NOTE: PIC advised food may have been our of cooler during breakfast service, but less than 2 hours. Discussed public health significance of minimizing time in the temperature danger zone with PIC.
  • Critical: Equipment / Food-Contact / Visibly Clean (corrected on site)
    Observation: The following equipment/utensils were observed soiled to sight and touch: can opener.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized. NOTE: PIC washed,rinsed, sanitized can opener during the inspection.

    Observation: There is no covered refuse container for the disposal of feminine napkins in the ladies room stall.
    Correction: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: raw chicken over pupusa dough in walkin cooler- food employee rearranged chicken to the bottom shelf. raw beef in the same container as bologna in turbo air 3dr preptop cooler, raw bacon over cooked bacon in 3dr preptop cooler-CFM removed and rearranged items.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc.
  • Gloves/Use Limitation/No Re-Use/Use For Only One Task (corrected on site)
    Observation: Single-use gloves worn during multi-tasked food preparation. food employee changed gloves during task.
    Correction: If used, single-use gloves shall be used for only ONE task and used for no other purposes. The single-use gloves shall be discarded when damaged or soiled or when interruptions occur in the operation.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: provolone cheese 45F in 3dr preptop cooler, milk 60F in carefes, raw beef 47F in turbo air 2dr cooler. cheese was put in prep cooler and raw beef was put in walkin cooler by CFM, milk was changed by food employee.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site) (repeated violation)
    Observation: When tested, the chlorine-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 3-vat sink with a concentration of 10 ppm total chlorine. Food employee corrected solution to 50 ppm.
    Correction: A chlorine-based sanitizing solution shall have a minimum concentration of at least 50 to100 ppm total chlorine when tested using the appropriate test kit.
03/07/2013Risk Factor
The purpose of this visit was to perform a complaint investigation in conjunction with a risk-factor assessment inspection. On 3/5/13, complainant allegedly saw food employee handle raw chicken with gloved hands and did not change their gloves and also saw the food employee put on another glove over the contaminated glove on one hand and touch cooked chicken and also did not observe any hand washing at the time. EHS observed food employees handling many tasks and saw a food employee had one gloved hand while doing multiple tasks and did not change their gloves in the beginning of inspection. CFM corrected food employees. EHS reminded CFM to make sure food employees are changing gloves and wash their hands OFTEN and to utilize utensils such as tongs to minimize cross-contamination. Based on today's observations, complaint is confirmed.

The purpose of this inspection was to conduct a routine inspection. Water heater: State Sandblaster CSB826SFEX. EHS provided copy of employee health plan (EHP), and explained plan in detail. Public health significance of not having EHP was emphasized. Discussed public health significance of leaving food products at room temperature, and suggested methods to keep these foods out of the temperature danger zone. Discussed importance of providing a sanitizer test kit and testing sanitizing solutions to insure they are neither too low (inadequate sanitization), or too high (potential toxin.) Provided poster with raw animal food cooking temperatures.

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19. PIC disposed of out of temperature food.
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: 3 dr prep unit left at serving line.
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Sanitizer Test Kit Required
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a chlorine solution test kit.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site)
    Observation: When tested, the chlorine-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils.     Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: shelving used to store spices, and drink dispenser surface behind nozzles.
    Correction: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.
  • Equipment & Utensils / Air-Drying or Adequate Draining Required
    Observation: baking pans were found stacked while wet after cleaning and chemical sanitization.
    Correction: After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food.

  • Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
    Observation: The concentration of the chlorine solution was measured at >200ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of chlorine solution at 50-100 ppm. Verify concentration using the appropriate test kit.
