    Observation: Observed that hand soap was not provided at the hand washing sink next to the back entrance door.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands.--Soap provided.

  • Critical: Fish Served Raw RTE / Frozen for Parasite Destruction
    Observation: The following fish offered for service or sale in the ready-to-eat form was not frozen to ensure parasite destruction: ACME packaged smoked salmon.
    Correction: Before service or sale in ready-to-eat form, raw or partially cooked fish shall be: 1) frozen and stored at a temperature of -4°F or below for a minimum of 7 days in a freezer, or 2) frozen at -31°F or below until solid and stored at -31°F or below for a minimum of 15 hours, or 3) frozen at -31°F or below until solid and stored at -4°F or below for a minimum of 24 hours. CORRECTIVE ACTION: PROVIDE DOCUMENTATION FROM SUPPLIER THAT SALMON WAS FROZEN FOR PARASITE DESTRUCTION OR FARM RAISED AND PELLET FED. See note in comments below.

    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means.     Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Shell eggs, block butter and sliced Muenster cheese at 46F. Items were in Superior 3-Door Prep Cooler (w/Top) at front service counter
    Correction: cooler had internal ambient air temperature of 46F, and damages door gaskets.
  • Critical: Consumer Advisory Required for Raw or Undercooked Animal Food
    Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food items that may be served raw and/or undercooked: SALMON,EGGS AND OMELETS, HAMBURGERS, ROAST BEEF.

    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.   • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: sliced tomato in True 2 door prep cooler at grill, 44F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. CORRECTIVE ACTION: tomato was moved to walk-in cooler.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site) (repeated violation)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: flavored cream cheeses in large plastic containers in the Kolpak Walk-In Cooler #1 were not clearly and consistently marked
    Correction: some had "use-by" dates, others had no dates.

    Observation: The following ready-to-eat food that is made using a combination of ready-to-eat, potentially hazardous ingredients is not properly dated for discard using the date of the first-prepared ingredient: Cream cheese spreads.
    Correction: Refrigerated, ready-to-eat, potentially hazardous food (time/temperature control for safety foods) that combined with additional ingredients, or portions of the same food, must retain the date marking of the earliest or first prepared ingredient. CORRECTIVE ACTION: Explained to PIC need to label with both production and discard dates.
  • Food Temperature Measuring Devices, F Scale, Accurate within 2°F
    Observation: The food temperature measuring device located in the following cold or hot hold unit is not accurate in °F: Internal Ambient Air Thermometer in Delfield 2-door sliding display cooler in the front area.
    Correction: Food temperature measuring devices that are scaled only in Fahrenheit shall be accurate to +/- 2°F in the intended range of use. Replace Thermometer.
  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation (repeated violation)
    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: The caulking around the front handwashing sink was discolored and soiled.
    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues. CORRECTIVE ACTION: Clean existing caulk or re-caulk the sink.
  • Light Bulbs / Locations where Shielding is Required (repeated violation)
    Observation: One fixture containing fluorescent light tubes in the kitchen is not covered by a protective shield. .
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles. CORRECTIVE ACTION: Provide a protective shield.
