2) Repair the wall by the 2DR Randell Prep Cooler.

  • Food Storage / Exposed to Splash, Dust, etc.
    Observation: Food stored in a location where it is subject to splash, dust or other contamination as follows: OBSERVED PROTEIN POWERS STORED NEXT TO THE HANDSINK WHERE WATER CAN DRIP FROM THE HANDS INTO THE POWDER WHEN STAFF REACH FOR THE PAPER TOWELS.
    Correction: Food shall be protected from contamination by storing the food where it is not exposed to splash, dust or other contamination. DISCUSSED POTENTIAL CONTAMINATION ISSUE WITH THE CFM. CFM WILL MOVE THE PAPER TOWEL DISPENSER TO A DIFFERENT LOCATION. PLEASE EMAIL A PHOTO OF THE HANDSINK AREA TO THE HEALTH DEPARTMENT AFTER REARRANGING THE EQUIPMENT.
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F
    Observation: Water from the handwashing sink at JUICE MACHINE was measured at a temperature less than 100°F. TEMPERATURE WAS MEASURED AT 97F.
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet. PLEASE EMAIL A COPY OF THE SERVICE REPORT TO THE HEALTH DEPARTMENT.
  • Critical: Air Gap Design Standard, Not Less than 1'
    Observation: An air gap between the water supply outlet and the flood rim level AT THE MOP SINK is not observed.
    Correction: An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch. PLEASE EMAIL A PHOTO OF THE MOP SINK TO THE HEALTH DEPARTMENT ONCE CORRECTED.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Food on Display Protected From Contamination
    Observation: The food on display is not protected from contamination- varrious powders for drinks by handsink
    Correction: Food on display shall be protected from contamination by the use of packaging, counter, service line, or salad bar food guards, display cases, or other effective means.
  • Cutting Boards / Resurface / Discard
    Observation: The cutting board(s) are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs.
    Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.

    Observation: Observed a hose attached to mopsink. The hose extended below the flood rim level of the sink basin.
    Correction: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and backflow prevention is required by Law by providing an air gap as specified under 5-202.13. CFM HUNG UP HOSE AT MOPSINK

  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 3 vat sink with a concentration of 0 ppm total quaternary ammonium compound. CFM corrected quat solution to 200 ppm.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.
  • Equipment / Non-food Contact / Visibly Clean
    Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: true 2dr cooler
    Correction: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
  • Handwashing Signage at Handwashing Facilities Required (corrected on site)
    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at bathroom handwashing sink used by food employees.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands.
  • Dressing Rooms and Lockers to be Used for Clothing Changes
    Observation: Employees are not using the dressing rooms or lockers provided.
    Correction: Instruct employees to use the designated dressing rooms to prevent contamination of food, food equipment and utensils, and preparation surfaces.
  • Critical: Conditions of Use/Presence & Use of Toxics Per Law (corrected on site)
    Observation: Pesticides labeled for residential use were observed in the facility and are being applied by the operator to control pests: can of roach spray. cfm discarded item
    Correction: Remove residential use pesticides from the facility. When necessary for pest control, chemical pesticides must be applied by a certified pesticide applicator to prevent injury and contamination.

The purpose of this visit was to respond to a complaint received by the Health Department on July 31, 2012. The complainant alleged the presence of moldy food and mold around the store. Upon investigation of the food and the food contact surfaces, no mold was observed.
The complaint is not confirmed.
