    Observation: Observed employee put on gloves without washing hands after smoking a cigarette.
    Correction: Instructed employee to wash hands.

    Observation: When tested, QAC sanitizer at 3-compartment sink (used to fill up buckets) measured >400 ppm.
    Correction: Sanitizer solution was diluted with water (400ppm reading) until the dispenser can be recalibrated. Sanitizer should be between 200-400ppm.

Observations discussed with PIC. Floor is in poor repair. Dish machine servicer was called during inspection- they will be coming out shortly to refill dish machine sanitizer, check the quat sanitizer at the 3-compartment, and bring new test strips. Continue to clean facility. Will follow-up regarding sanitizer concentrations.

    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a quat test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Critical: Warewashing - Manual & Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration & Hardness*
    Observation: Quat sanitizer concentration was above 500 ppm.
    Correction: Adjust the sanitizing solution to 200 parts per million or otherwise identified by manufacturer's recommendations.
  • Critical: Warewashing - Manual & Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration & Hardness*
    Observation: Sanitizer 0 ppm. Sanitizer container was nearly empty. Switched to a new container, but the sanitizer isn't moving through the line properly. A switch to prime the sanitizer line could not be located.
    Correction: Contact the servicer to come out and restore proper sanitizer levels.
  • Non-Food Contact Surfaces
    Observation: The following were observed: dusty ceiling, dirty shevling throughout kitchen, moldy gasket in the back freezer, dirty mop sink, greasy floor behind fryer.
    Correction: Clean.

    Observation: Found torn and damaged gaskets on cold hold units.
    Correction: All equipment to be in good condition. Please repair/replace gaskets.
  • Backflow Prevention Device, Location (corrected on site)
    Observation: Found non-pressurized backflow by design in a pressurized location on mop sink faucet.
    Correction: Backflow units to be located in proper location. Please place backflow unit properly. Hose was taken off.

Corrective actions must be submitted to me by 4/15/13. Items that must be corrected: 1. cleaning 2. light intensity 3. light covers and/or shields.
