    Observation: The food employee observed eating food on food prep table where they may contaminate food, clean equipment, utensils or other items needing protection.
    Correction: Operator voluntarily instructed the employee to eat in the designated dining area.
  • Records - Creation and Retention
    Observation: No parasite destruction records maintained for raw, raw marinated, partially cooked or marinated-partially cooked fish that are served or sold in ready-to-eat form.
    Correction: Obtain a written agreement or statement from the supplier stipulating that the fish supplied are frozen throughout to a temperature of -4°F (-20°C) for 168 hours (7 days) in a freezer or at -31°F (-35°C) for 15 hours in a blast freezer.
  • Handwashing - Using a Handwashing Lavatory (corrected on site)
    Observation: The only handwashing facility located at the kitchen is blocked, preventing access by employees for easy handwashing.
    Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Operator voluntarily removed the wiping cloth, detergent and handwashing soap preventing its use.

    Observation: Open food employee observed drinking from an uncovered container in the sushi preparation area.
    Correction: Employees may drink from a closed beverage container if the container is handled to prevent contamination of (1) The employee's hands, (2) The container, and (3) Exposed food, clean equipment, utensils, and linens, and unwrapped single-service and single-use articles.

    Observation: The prepared ready-to-eat (RTE) food held for 3 days in the walk in refrigeration unit is not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: Employees observed handling ready-to-eat (RTE) food with their bare hands in that the cook was observed touching ready to eat steam vegetables with bare hands when placing them in plates to serve.
    Correction: Provide suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment to handle RTE food to prevent contamination from hands. Operator voluntarily reheated said food to 165 degrees F.

    Observation: Raw foods of animal origin stored over ready-to-eat (RTE) cut vegetables in the refrigeration unit.
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
  • Handwashing Lavatory, Water Temperature, and Flow (corrected on site)
    Observation: Water from the handwashing sink at sushi prep area was measured at a temperature less than 100°F in that hot running water not provided in said handwashing facility..
    Correction: Make necessary adjustments to valves and lines serving the handsink to provide water of at least 100°F to allow more effective removal of fatty soils encountered in kitchens. Both hot and cold running water must be provided in all handwashing facilities.
  • Hand Drying Provision
    Observation: No disposable towels were provided at the hand washing lavatory in the men's restroom.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

    Observation: There is no handwash lavatory in kitchen preventing routine handwashing by food workers in that two handwashing sinks in kitchen observed without running hot/cold water. Also said sinks were blocked during inspection.
    Correction: Operator voluntarily designated one of the sink of 3 compartment sinks only for handwashing. Facility has low temperature chemical dishwasher being used to wash, rinse and sanitize dishes. Handwashing facility must be repaired and maintained in good repair.
