Today I conducted a routine inspection and discussed the following with the certified food manager:
1. Management stated all seafood items are fully cooked to an internal temperature of 145f so Parasite Destruction information is not required.
2. Management will continue to stir and cover foods in the steam well to keep temperatures at 135f or above at all times.
3. Management should calibrate all food thermometers at least once a week in an ice water bath to 32f to ensure accuracy.
4. Management will ensure menus are accurately reflecting the Consumer Advisory when any changes are made.

  • Critical: Handwashing / When to wash hands (corrected on site)
    Observation: Observed food employees failing to wash their hands before putting on clean, single use gloves.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness. DISCUSSED WITH MANAGER. MANAGER SHALL RETRAIN STAFF AND MONITOR HAND WASHING PRACTICES.
  • Sanitizer Test Kit Required
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a QUATERNARY AMMONIA test kit to ensure the concentration of sanitizer is being maintained at 150-400ppm or per manufacturer's specifications in the sanitize basin of the 3-vat sink and red sanitizer buckets. DISCUSSED WITH MANAGER. MANAGER HAS A TEST KIT BUT IT HAS BEEN EXPOSED TO MOISTURE SO HE WILL ORDER A NEW ONE.

    Observation: The following food item(s) were found hot holding at improper temperatures using a calibrated food temperature measuring device DICED POTATOES ON STEAM TABLE AT 109f:
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be hot held at a temperature of 135°F or above unless the permit holder is using "time as public health control" as specified under 3-501.19. REHEATED TO 182F ON THE GRILL
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item was observed cold holding at improper temperatures using a calibrated food temperature measuring device: ROAST BEEF SANDWICH 48F IN SELF SERVE DISPLAY
    Correction: Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below. DISCARDED

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: mozzarella and sliced tomato on cold table at 49F and 47F on cold table.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. COLD TABLE WAS HOLDING AT 28F HOWEVER FOOD ITEMS WERE BEING DOUBLE-PANNED. FOOD ITEMS WERE DISCARDED.
  • Critical: Consumer Advisory Required for Raw or Undercooked Animal Food (corrected on site)
    Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food items that may be served raw and/or undercooked: burgers, eggs.
    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs.
    Observation: Interior of ice bin was observed in need of cleaning.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.

  • Duties / Monitor Sanitizing Practices and Procedures (corrected on site)
    Observation: Employees are not properly sanitizing cleaned multiuse equipment and utensils. Observed food employee washing in the sanitize compartment. Three-vat sink was observed set up with sanitizer in the rinse compartment.
    Correction: The Person in Charge or certified food manager shall train his/her employees to properly sanitize all equipment, utensils, and surfaces after being cleaned using either an approved sanitizer at the adequate concentration or heat sanitization at the proper temperature to destroy pathogenic bacteria. TRAINING PROVIDED TO FOOD EMPLOYEE DURING TODAY'S VISIT.
  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. Forms have been provided to establishment, however only unsigned forms were available for health department review during today's visit.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Wiping Cloths / Use Limitation /Dry
    Observation: Observed towels under cutting board being used to prevent sliding.
    Correction: Cloths shall not be used for any other purposes. For example, cloths shall not be placed under cutting boards to prevent the boards from sliding while in use.
  • Sanitizer Test Kit Required (repeated violation)
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a QUATERNARY AMMONIA test kit.
  • Permit to Operate Required, Must be Posted, Must be Valid
    Observation: The establishment has not posted the permit to operate in a conspicuous location.
    Correction: No person shall operate a food establishment unless that person possesses a valid permit issued by the Director. Only a person who complies with the requirements of this Chapter shall be entitled to receive or retain such a permit. Permits are not transferable from one owner to another. A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue.
  • Handwashing Sink / Accessible at All Times (corrected on site)
    Observation: The handwashing facility located in the front kitchen is blocked with kitchen utensils, preventing access by employees for easy handwashing.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use. KITCHEN UTENSILS BLOCKING HANDSINK WERE REMOVED.
  • Plumbing / Maintained in Good Repair
    Observation: Plumbing connections under the 3-vat sink piping are leaking.
    Correction: A plumbing system shall be maintained in good repair.
  • Physical Facilities Good Repair
    Observation: Observed that the wall structure by the plumbing at the back handsink is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Sliced tomatoes in cold-holding wells at 50F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. REMOVED FROM CONTAINER AND MOVED TO COOL TO 41F.
  • Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site) (repeated violation)
    Observation: The concentration of the quaternary ammonia solution was measured at 500 ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of quaternary ammonia solution between 150-400 ppm. Verify concentration using the appropriate test kit. SANITIZER AT 150 PPM WAS PROVIDED.
