The purpose of today's visit was to conduct a risk factor assessment. Observed good employee hand washing during inspection. Please obtain a Certified Food Manager card and email or fax a copy to the Health Department within 30 days.
  • Certified Food Manager/Certificate Process (repeated violation)
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.

  • Refuse Container with Cover/ Women's & Unisex Toilet Room
    Observation: There is no cover to the feminine napkin refuse container in the ladies room stall.
    Correction: A toilet room used by females shall be provided with a covered receptacle. OBTAIN A COVERED TRASH CAN.
03/19/2015Routine
The purpose of this visit was to conduct a risk factor inspection.
Manager stated that employee health training is conducted every month.
Environmental health specialist (EHS) observed Artic Air 1dr cooler holding at 50 degrees. The unit has been opened and a delivery had just arrived. EHS recommended that manager document the unit temperatures in the morning to make sure that the unit is holding at proper temperature. EHS will follow-up on the unit and check the foods to make sure that the unit and foods are holding at 41 degrees and below by August 12, 2014.
EHS recommended that a refrigerator be provided for personal foods for employees to prevent contamination and provide additional space in the unit.

No violation noted during this evaluation.

The purpose of this visit was to conduct a routine inspection.
Environmental health specialist (EHS) discussed having the proper thermometer inside of the refrigeration units. EHS recommended that managers or staff document refrigeration unit temperatures as well as reheating temperatures.
EHS discussed the proper methods to thaw foods when preparing meals.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: sliced turkey, mac and cheese holding at 48 degrees inside of Artic Air 1dr cooler
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. All food should be discarded and when shipment is received all food items should be stored in the freezer until the refrigerator is able to hold proper temperature.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Artic Air 1dr cooler
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.

EHS recommended documenting reheat temperatures and providing a refrigerator thermometer inside of the Arctic Air unit.

The purpose of this visit was to conduct a routine inspection.
Manager will fax over a copy of training documentation for employee health by March 25, 2013.
EHS discussed storage of personal food in refrigerators, cleaning schedule and datemarking.

    Observation: Absence of no-smoking signs or international no-smoking signs in non-smoking area.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (repeated violation)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
